- CONCLUSIONS: ISP with viral pneumonia represents a clinically complex, high-risk group requiring individualized treatment strategies, including antiviral agents, vigilant safety monitoring, and proactive preventive measures, especially vaccination. Virologic clearance benefits associated with direct-acting antivirals such as remdesivir do not consistently translate into a short-term mortality benefit, underscoring the need for outcome- specific interpretation of the evidence. A nuanced understanding of the available antiviral armamentarium is essential to optimise outcomes in this vulnerable population.

- Clinical Outcomes of Respiratory Syncytial Virus Versus Influenza Pneumonia in Critically Ill Adults. [Journal Article]
- CONCLUSIONS: RSV pneumonia was associated with higher in-hospital mortality and a longer ICU stay than influenza pneumonia in this ICU cohort. The mortality association remained after adjustment for the variables included in the primary model, although its magnitude was sensitive to model specification. Given the observational design, these findings should not be interpreted as causal. They underscore the clinical burden of severe RSV infection in adults and support further investigation in larger cohorts with standardized virological testing and measures of acute illness severity.

- Baloxavir marboxil versus oseltamivir for influenza A in children aged 5 years and older: a community-based prospective cohort study. [Multicenter Study]
- CONCLUSIONS: In community-dwelling children with influenza A, baloxavir marboxil was associated with more rapid fever resolution than oseltamivir. Exploratory findings suggested potential benefits regarding viral clearance, complications, healthcare utilization, and tolerability. Because treatment selection was preference-based and open-label, and because influenza A subtype and reduced-susceptibility substitutions were not evaluated, these findings should be interpreted as hypothesis-generating rather than causal.
