- Trends in Pseudomonas aeruginosa Resistance After a Four-Dimensional Antimicrobial Stewardship Intervention. [Journal Article]J Vis Exp. 2026 Oct 13; (236).JV
- This single-center retrospective study analyzed de-duplicated Pseudomonas aeruginosa isolates collected at Huaian Municipal Infectious Diseases Hospital from 2023 to 2025. A total of 121 unique isolates were included, of which 83.5% were from sputum. Antimicrobial susceptibility testing (AST) was performed using an automated microbiology system, and minimum inhibitory concentration (MIC) results …

- Omitting the Eradication Phase of Treatment of Adults with Melioidosis: A Pilot, Single-Center Randomized Controlled Trial. [Journal Article]Open Forum Infect Dis. 2026 Oct; 13(10):ofag591.OF
- CONCLUSIONS: Prolonged intravenous antibiotics alone will cure many patients with melioidosis; however, some still require oral eradication therapy. The potential for recurrence of life-threatening melioidosis must be balanced against the high rate of significant ADRs to high-dose TMP/SMX. This necessitates a greater understanding of the individuals at highest risk of recurrence and determination of the optimal dosing and duration of eradication therapy in those who need it.

- High Rate of Adverse Events in Patients Receiving Oral Antimicrobials Under a COpAT Monitoring Protocol. [Journal Article]Open Forum Infect Dis. 2026 Oct; 13(10):ofag590.OF
- We conducted a retrospective review to characterize adverse events in patients receiving oral linezolid, trimethoprim-sulfamethoxazole, voriconazole, and itraconazole monitored under an outpatient parenteral antimicrobial therapy program. Among 118 patients, 61% incurred at least 1 adverse event during treatment, highlighting the need for evidence-based monitoring protocols for complex outpatient…

- A case report of ocular nocardiosis secondary to Nocardia asiatica in a patient with acquired immunodeficiency syndrome. [Case Reports]Am J Ophthalmol Case Rep. 2026 Dec; 44:102674.AJ
- CONCLUSIONS: This case represents a detailed report of endogenous ocular nocardiosis attributable to Nocardia asiatica presenting as bilateral subretinal abscesses with multifocal brain involvement. Subretinal abscess-like lesions with minimal intraocular inflammation in the setting of disseminated infection, particularly when accompanied by multifocal brain abscesses, should prompt early consideration of nocardiosis. Early recognition and empiric trimethoprim-sulfamethoxazole therapy may facilitate systemic recovery and preserve vision in less affected eyes.

- Burkholderia pseudomallei, the causative agent of melioidosis, is a gram-negative bacterium historically endemic to Southeast Asia and Northern Australia. However, recent environmental detection along the Gulf Coast and an increasing number of reported cases in non-travelers have raised questions regarding potential endemicity and environmental establishment in the Southern United States. We repo…

[Journal Article]Infect Dis Ther. 2026 Oct 12. [Online ahead of print]ID

- Acute Inner Foveal Toxoplasmic Maculopathy: An Atypical Manifestation of Ocular Toxoplasmosis. [Journal Article]Ocul Immunol Inflamm. 2026 Oct 12; :1-7. [Online ahead of print]OI
- CONCLUSIONS: This case series described an atypical clinical and imaging presentation of ocular toxoplasmosis characterized by a distinctive inner foveal SD-OCT imaging pattern (acute inner foveal toxoplasmic maculopathy). Although the absence of intraocular microbiological confirmation limits diagnostic certainty, recognition of this presentation may facilitate earlier diagnosis and appropriate treatment in selected patients. The term "acute" refers to the active ocular presentation and does not imply recently acquired systemic infection. Further studies are warranted to validate these findings and clarify their underlying pathophysiology.

- Use of direct injection UHPLC-MS/MS for antibiotic population use assessment by wastewater analysis - application to laboratory and monitoring studies. [Journal Article]Anal Methods. 2026 Oct 12. [Online ahead of print]AM
- Antimicrobial resistance (AMR) is a global human health threat and wastewater analysis of antibiotics can help inform AMR policies. Accurate quantification of antibiotic use in urban populations is needed. This requires analytical methods capable of high sample throughput to support short-term laboratory antibiotic fate investigations where large sample numbers are generated, and sub-µg L[-1] qua…

- A Six-Year Retrospective Surveillance of Outpatient Staphylococcus aureus Resistance Dynamics in Skin and Soft Tissue Infections: Are Non-β-Lactam Options Still Reliable? [Journal Article]Infect Dis Clin Microbiol. 2026 Sep; 8(5):472-481.ID
- CONCLUSIONS: Rising MRSA prevalence and decreasing susceptibility to clindamycin and tetracycline may limit empirical oral treatment options for outpatient SSTIs. TMP-SMX susceptibility remained stable in vitro. Continuous outpatient-focused surveillance is essential to support evidence-based empirical therapy.

- Pasteurella multocida, a Gram-negative coccobacillus commonly found in the oropharyngeal flora of cats, is a rare cause of peritoneal dialysis-associated peritonitis. We report the case of a 44-year-old patient with exposure to stray cats who presented with abdominal pain and cloudy dialysis effluent. Laboratory tests showed leukocytosis and elevated C-reactive protein (CRP), and we initiated emp…

- Reducing antibiotic burden in gram-negative bacteraemia: duration optimization and early oral transition. [Journal Article]Curr Opin Infect Dis. 2026 Oct 12. [Online ahead of print]CO
- CONCLUSIONS: Current evidence supports reducing antibiotic exposure in selected patients with Gram-negative bloodstream infections by combining shorter treatment durations and early oral step-down therapy. Future studies should address remaining evidence gaps, particularly in frail patients.

- Mutation-induced conformational remodeling of dihydropteroate synthase governs sulfonamide resistance in Streptococcus pneumoniae. [Journal Article]Environ Res. 2026 Oct 10; 309(Pt 2):125894. [Online ahead of print]ER
- Streptococcus pneumoniae is a major bacterial pathogen, and dihydropteroate synthase (DHPS) is the primary target of sulfonamides (SAs). Environmental exposure to SAs promotes the emergence and spread of antibiotic resistance. However, the molecular basis of DHPS mutation-mediated resistance remains unclear. Derived from the folP sequences of trimethoprim-sulfamethoxazole-resistant clinical isola…

- Evaluation of biofilm activity, virulence genes, and antibiotic susceptibility profiles in clinical Proteus mirabilis isolates. [Journal Article]J Infect Dev Ctries. 2026 Sep 30; 20(9):1351-1360.JI
- CONCLUSIONS: Biofilm-mediated resistance is a significant barrier to managing P. mirabilis infections, particularly in wounds and catheter-associated UTIs. The presence of specific genes in moderate-to-strong biofilm producers indicates their products are integral to the structural integrity and the functional capacity of the biofilm. The inverse relationship between rsbA and carbapenem resistance warrants further investigation as a biomarker for susceptibility.
