  • CONCLUSIONS: In this cohort of women with UI, rural women face a compounding set of psycho-socio-financial burdens of their urinary symptoms, despite similar UI severity as compared to non-rural women. Effective care for this population requires a multi-disciplinary approach beyond the clinic by integrating telehealth, social work, financial counseling, and community-based resources into management of UI.

  • CONCLUSIONS: At 1 year, sling surgery is the costliest first-line strategy for SUI. Pessary and PFMT demonstrate lower average costs. These findings support cost-conscious, evidence-based care pathways that prioritize conservative management as initial therapy, while acknowledging that cost minimization does not capture effectiveness or durability. Future research incorporating longer time horizons, bulking agents, and prospective utility measurement is needed for a full cost-effectiveness evaluation.

  • CONCLUSIONS: Voiding symptoms and flatal incontinence frequently co-occurred in this axSpA cohort, and anxiety was independently associated with flatal incontinence. These hypothesis-generating findings raise the possibility of a broader sacropelvic symptom cluster in axSpA extending beyond the sacroiliac joints. Given the cross-sectional design and the absence of a control group, no causal or mediating role of psychological distress can be inferred.
