- Predictive Modeling of Clinically Apparent Shoulder Periprosthetic Joint Infection (PJI). [Journal Article]J Shoulder Elbow Surg. 2026 Oct 12. [Online ahead of print]JS
- CONCLUSIONS: Using prospective, multicenter data, this study proposes a seven-factor nomogram to estimate the probability of clinically apparent shoulder PJI. The model showed strong apparent discrimination for clinically apparent infection in the development cohort. External validation will be necessary to confirm performance, and consensus will be required to establish weighting and diagnostic thresholds for tiers of non-definite shoulder PJI.

- Forearm Flexor Strain and Subsequent Ulnar Collateral Ligament Surgery Risk in Major League Baseball Pitchers: A Covariate-Matched Survival Analysis. [Journal Article]Am J Sports Med. 2026 Sep 28; :3635465261486103. [Online ahead of print]AJ
- CONCLUSIONS: MLB pitchers returning after a nonoperative forearm flexor strain have elevated short-term risk of subsequent UCL surgery versus matched controls within the first year. Return timing was the primary measurable predictor, with longer recovery associated with progressively lower UCL surgery rates. However, because strain grade, UCL integrity, and associated medial elbow pathology were unavailable, RTP timing should be interpreted as a risk marker rather than evidence that delayed return is independently protective.

- Clinical and radiographic outcomes following medial collateral ligament-sparing interposition elbow arthroplasty using acellular dermal allograft. [Journal Article]
- CONCLUSIONS: Interposition elbow arthroplasty using an acellular dermal allograft with a medial collateral ligament-sparing approach yields improvements in pain and function that reach a minimally clinically important difference. Patients undergoing revision or secondary surgery experienced very high rates of complication or reoperation, while those undergoing primary surgery did not. Despite partial loss of early radiographic gains, the procedure does not appear to accelerate degenerative changes, supporting its role as a joint-preserving option in younger patients.

- Sidearm Deliveries Are Associated With Lower Shoulder and Elbow Injury Burden and a Favorable Performance Profile in Major League Baseball: A Cohort Study of 927 Pitchers (2020-2025). [Journal Article]
- CONCLUSIONS: In this MLB cohort, sidearm deliveries were associated with lower shoulder/elbow time-loss burden, superior pitch quality, and lower hard-hit rates. Shoulder injury burden increased linearly with more overhand deliveries, whereas elbow burden followed a nonlinear pattern that peaked within the overhand range. These associations were independent of workload, velocity, and pitch quality. Arm slot may represent a clinically relevant marker of pitcher injury risk that warrants prospective study.

- Quality and rate of recovery after Simpliciti anatomic total shoulder arthroplasty and Tornier Pyrocarbon hemiarthroplasty: a propensity score‑matched comparative study. [Journal Article]J Shoulder Elbow Surg. 2026 Jul 28. [Online ahead of print]JS
- CONCLUSIONS: Simpliciti aTSA and Tornier PyroC-HA are effective surgical treatments for younger patients with GHOA with similar outcomes at short-term follow-up. This study demonstrated that patients experienced similar recoveries and nearly identical 2-year outcomes, with the exception of a statistically significant, less than 1 point difference in pain at 24 months post-operatively.

- Ulnar Neuropathy at the Elbow: Evaluation of Discriminatory Performance of Multitiered Electrodiagnostic-Based Severity Classifications. [Journal Article]J Clin Neurophysiol. 2026 Sep 25. [Online ahead of print]JC
- CONCLUSIONS: Multitiered EDX-based classifications of UNE severity demonstrate poor discrimination between adjacent categories. Consolidated binary categorizations of UNE provide superior discriminatory capability and may offer greater clinical utility. These findings apply specifically to electrophysiologic assessment and should not be interpreted as measures of clinical symptom severity. Sex and age category should be considered when interpreting EDX results in patients with suspected UNE.

- Functional and radiographic outcomes of allograft reconstruction for contained glenoid defects in revision reverse total shoulder arthroplasty: a case series. [Journal Article]J Shoulder Elb Arthroplast. 2026 Sep; 10(3):100054.JS
- CONCLUSIONS: In the short to mid-term, revision rTSA with allograft reconstruction for contained central glenoid defects using a long-post baseplate provides excellent radiographic healing, improved range of motion, good function, low pain, and low baseplate revision rates. This technique may represent an effective option for managing contained central glenoid defects in the setting of revision rTSA.

- PERC-Peds rule for bedside exclusion of pulmonary embolism without radiation in children in the USA (BEEPER): a multicentre, prospective, observational, diagnostic accuracy study. [Multicenter Study]
- CONCLUSIONS: In this multicentre, prospective, observational, diagnostic accuracy study of children with suspected pulmonary embolism in the emergency department, we found a 6·3% prevalence of pulmonary embolism or proximal DVT; in this population, the PERC-Peds negative rule can safely rule out pulmonary embolism. Use of PERC-Peds might reduce low-value diagnostic testing for pulmonary embolism in children and adolescents.

- Pediatric Supracondylar Humerus Fracture Pinning: Are We Leaving Unnecessary Waste and Cost on the Table? [Journal Article]J Pediatr Soc North Am. 2026 Aug; 16:100384.JP
- CONCLUSIONS: CRPP surgery for SCH fractures generates substantial waste and emissions. This LCA demonstrates that a semi-sterile setup substantially reduces waste and emissions, particularly as it relates to disposable drapes, gowns and packaging. Minimizing waste reflects thoughtful resource and environmental stewardship, with meaningful benefits to the health of children now and in the future. Such efforts also reduce supply and disposal costs, with ramifications for improving the value of care for this common injury.

- Influence of radiographic viewing perspective on the reverse shoulder arthroplasty angle. [Journal Article]
- CONCLUSIONS: The 2D RSA angle experiences viewing perspective errors when not measured on a true AP radiograph. The glenoid midpoint provided the most consistent and smallest maximum error, with good to excellent reliability. The 3D underestimated the 2D RSA angle on true AP images by an average of -8.0°, with similar variability. When using 2D imaging, a true AP image is desirable; otherwise, use the glenoid midpoint to minimize viewing perspective errors. Comparisons between radiographic measures and 3D preoperative planning should consider the bias between the two techniques, and 3D measures should be evaluated for their measurement techniques within the respective preoperative planning softwares to ensure consistency and reliability among manufacturers.

- Return to sport after ulnar collateral ligament repair and reconstruction. [Journal Article]J Shoulder Elbow Surg. 2026 Nov; 35(11):2778-2785.JS
- CONCLUSIONS: Both UCL reconstruction and repair are successful treatment options for UCL injuries in overhead athletes with high return to play rates. Repair was associated with a 0.9-month quicker return to throwing and 1.7-month quicker return to play and lower rates of postoperative ulnar neuritis, but a higher complication rate of 10% compared to 3% in the reconstruction cohort. However, these findings are purely associations given the different indications for each procedure.
