- Beyond the classic ketogenic diet: A systematic review of alternative and adjunctive therapies for GLUT1 deficiency syndrome. [Review]Epilepsy Res. 2026 Sep 23; 228:107926. [Online ahead of print]ER
- CONCLUSIONS: While CKD remains the reference standard for GLUT1-DS management, selected alternative and adjunctive therapies may offer clinically meaningful benefit in specific clinical contexts. The current evidence base, however, is limited. These findings support cautious and individualized use of alternative therapeutic strategies while highlighting the need for larger prospective studies and standardized outcome measures to better define their role in GLUT1-DS management.

- Efficacy and Tolerability of Zonisamide for Alcohol Use Disorder: A Systematic Review and Meta-Analysis. [Journal Article]J Clin Psychopharmacol. 2026 Sep 18. [Online ahead of print]JC
- CONCLUSIONS: Zonisamide reduced drinks per day and drinking days but not heavy drinking days, suggesting a selective benefit on overall consumption. A protective effect on nervousness, anxiety, and irritability was also observed, consistent with zonisamide's GABAergic mechanism. Further well-powered trials are needed to inform clinical recommendations.

- Trends in Use of Antiseizure Medicines in Women With Epilepsy in Pregnancy During 2018-2023 in an Irish Maternity Hospital: A Cohort Study. [Journal Article]
- CONCLUSIONS: In this single-centre EHR cohort, trends in use of ASMs in WWE appeared relatively stable, although analyses of less commonly used ASMs were limited by very small numbers. ASMs with safer pregnancy profile were most frequently used in WWE in pregnancy. Additional data are needed to further assess trends and outcomes and to determine the place of newer generation ASMs in WWE in pregnancy.

- Potential Role of CYP3A4 in Determining In Vivo Exposure to Cannabidiol (CBD) and its Active Metabolite 7-OH-CBD: Evidence from an In Vitro Study. [Journal Article]
- CONCLUSIONS: ASMs with CYP3A4-inhibitory potential may alter systemic exposure to both CBD and its active metabolite, 7-OH-CBD, as demonstrated in vitro. However, co-administration of CBD with CYP3A4-substrate ASMs or CYP2C19 inhibitors is predicted to result in only modest increases in CBD exposure.

- Therapeutic drug monitoring of zonisamide in children with epilepsy: development and validation of a precise binary prediction model. [Journal Article]
- CONCLUSIONS: The machine learning prediction model constructed using the CART algorithm can accurately and stably predict whether steady-state trough concentrations of zonisamide reach the target therapeutic range in pediatric patients with epilepsy.

- Zonisamide attenuates morphine tolerance in association with reduced spinal TLR4/p38 MAPK signaling. [Journal Article]
- Morphine tolerance limits its long-term clinical utility, and neuroinflammation is a key underlying mechanism. This study investigated whether zonisamide, an antiepileptic drug with anti-inflammatory properties, attenuates morphine-induced neuroinflammation and morphine tolerance.

- Cognitive Side Effects of Antiseizure Medications in Adults with Epilepsy: An Update with a Focus on New Therapeutic Agents. [Review]
- Antiseizure medications (ASMs) are the mainstay of epilepsy treatment but may adversely affect cognitive functions, deepening the cognitive and psychosocial burden intrinsic to epilepsy. Cognitive side effects vary widely across drug classes, doses, treatment regimens, and individual susceptibility, typically affecting attention, processing speed, memory, language, and executive function. This na…

- Case Report: SMART syndrome mimicking Todd's paralysis after meningioma irradiation. [Case Reports]
- Stroke-like Migraine Attacks after Radiation Therapy (SMART) syndrome is a rare, delayed complication of cerebral irradiation characterized by stroke-like symptoms, seizures, and/or headaches, with mean latency period of 11.2 years. It is most associated with gliomas and medulloblastomas and diagnosis is challenging due to symptom overlap with recurrent neurologic deficits. We present a 59-year-o…

- Speech-Induced Myoclonus-Dystonia Mimicking Stuttering and Responding to Zonisamide. [Journal Article]Ann Indian Acad Neurol. 2026 Aug 28. [Online ahead of print]AI
- We present the case of a child with myoclonus-dystonia (MD) who was mistaken for having stuttering and responded to oral zonisamide therapy. An 11-year-old boy presented with a 3-month history of speech-induced myoclonus, dystonia, and speech disfluency, with a positive family history in his father. A final diagnosis of MD was made, and the child responded to zonisamide. MD is a treatable form of…

- Infertility treatment in women with epilepsy: A systematic review. [Systematic Review]Epilepsy Behav. 2026 Nov; 184:111210.EB
- CONCLUSIONS: Available evidence suggests that ART is feasible in women with epilepsy, with most patients maintaining stable seizure control. Hormonal therapy may affect ASM pharmacokinetics and seizure threshold, thereby warranting close monitoring. Larger prospective studies are needed to better define ASM-specific effects and optimize care.

- Carbonic anhydrase (CA, EC 4.2.1.1) isoforms I (CA I) and II (CA II) are widespread cytosolic proteins in most vertebrates. They are abundant in the red blood cells and many other tissues, being involved in physiological processes such as pH regulation, CO₂/bicarbonate homeostasis, respiration, and secretion of electrolytes rich in acid or bicarbonate in the stomach, kidneys, cerebrospinal fluid,…
