- Aquablation versus laser enucleation of the prostate for benign prostatic hyperplasia: a systematic review and pairwise meta-analysis of head-to-head studies including the first randomized trial. [Systematic Review]Front Surg. 2026; 13:1937749.FS
- CONCLUSIONS: No statistically significant difference in overall symptom relief was observed between Aquablation and laser enucleation, though a non-significant numerical trend in large glands is exploratory and requires confirmation. Aquablation confers markedly better, size-independent ejaculatory preservation, while laser enucleation provides superior objective deobstruction. Certainty of evidence remains low to moderate given the limited partially randomized base and short follow-up. These findings support an individualized choice between procedures based on gland size and patient priorities.

- Reproductive postponement as a clinical problem: biological limits, reproductive difficulties and psychosexual impact on the couple. [Review]Cas Lek Cesk. 2026; 165(5):187-192.CL
- Reproductive postponement in contemporary Czechia is not only a demographic trend but also a clinically relevant challenge. Delaying parenthood increases the probability of prolonged time to pregnancy, reproductive difficulties, need for specialized fertility care and non-realization of reproductive intentions. This narrative review integrates Czech demographic evidence with clinical aspects of f…
- Topiramate Impairs Spermatogenesis, Sperm Parameters, and Epididymal Duct Contractility in Rats. [Journal Article]
- Topiramate is a widely prescribed antiepileptic drug with a multimodal mechanism of action. However, its effects on male reproductive function remain poorly characterized. This study was conducted to investigate the impact of this drug on testicular and epididymal function in adult male Wistar rats. Animals were treated orally for 21 days with topiramate at 40 or 160 mg/kg. Serum testosterone lev…

- Intracytoplasmic Sperm Injection Outcomes Using Epididymal Sperm Obtained by Percutaneous Aspiration: An 11-Year Single-Center Experience. [Journal Article]
- CONCLUSIONS: PESA combined with ICSI provides favorable reproductive outcomes with a low rate of total fertilization failure. These results were achieved despite a cohort characterized by diminished ovarian reserve and diverse male infertility etiologies. The high sperm retrieval success rate and consistent clinical outcomes support PESA as a reliable and effective sperm retrieval technique in contemporary assisted reproductive practice.

- Safety and functional efficacy of transperineal laser ablation in large prostates (>80 mL): a prospective single-center study. [Journal Article]Minerva Urol Nephrol. 2026 Aug; 78(4):583-591.MU
- CONCLUSIONS: In prostates >80 mL, TPLA under local anesthesia is feasible and safe, yielding delayed yet substantial improvements in LUTS and flowmetry from 6 months onward, low sphincter-related morbidity, and preserved ejaculation. TPLA may complement enucleation procedures as a volume-adapted, minimally invasive option for selected patients.

- Prostatic urethral lift in young men seeking preservation of ejaculatory function and fertility-related parameters: semen analysis up to 18 months and comparative review of MIST. [Journal Article]Minerva Urol Nephrol. 2026 Aug; 78(4):574-582.MU
- CONCLUSIONS: In carefully selected young men, PUL did not result in a deterioration of the semen quality and ejaculation through 18 months while providing durable symptom relief. To our knowledge, this is the first report of successful paternity following PUL. Ejaculation-sparing MIST should be central to counseling for fertility-conscious patients.

- Factors predicting the success of ejaculation preserving transurethral resection of the prostate: a prospective cohort study. [Journal Article]
- CONCLUSIONS: EP-TURP provides significant improvement in voiding parameters while preserving antegrade ejaculation in a substantial proportion of patients. Larger prostate size, significant intravesical prostatic protrusion, and median lobe enlargement are associated with lower preservation rates and should be considered during patient selection and preoperative counseling.

- The relationship between the severity of erectile dysfunction and the risk of co-occurrence with premature ejaculation: a cross-sectional study. [Journal Article]
- CONCLUSIONS: This cross-sectional study suggests an asymmetric association pattern between ED severity and PE risk, most evident during the mild-to-moderate progression. Because of the cross-sectional design, these findings are preliminary and should be interpreted as associations rather than causal relationships. Prospective studies are needed to determine whether early ED progression is a critical risk window for PE. The mild-to-moderate ED group, representing a larger target population with the most pronounced risk escalation, warrants further investigation in future studies.

- Temporary pharmacologically induced ejaculatory dysfunction influences surgical decision-making in sexually active men with benign prostatic hyperplasia: a prospective interventional study. [Journal Article]
- CONCLUSIONS: Temporary pharmacologically induced ejaculatory dysfunction was associated with a significant change in treatment preference in a substantial proportion of sexually active men undergoing counseling for BPH surgery. These findings suggest that experiential exposure to reversible ejaculatory dysfunction may represent a novel, hypothesis-generating decision-support strategy, complementing conventional counseling. Further studies are needed to validate this approach and to assess its impact on actual treatment selection and long-term satisfaction.

- Ejaculation sparing versus standard technique in bladder outlet obstruction procedures: a systematic review, meta-analysis, and trial sequential analysis. [Systematic Review]J Sex Med. 2026 Sep 14; 23(10).JS
- CONCLUSIONS: Ejaculation-sparing techniques conferred greater preservation of AE compared with standard BPH procedures, without adversely affecting urinary or sexual functional outcomes at short-term follow-up. These findings support consideration of ES techniques in appropriately selected patients for whom preservation of sexual function is a priority. Further prospective studies with standardized surgical definitions and longer follow-up periods are warranted to determine the long-term efficacy and durability of these approaches.
