- Extracorporeal Cardiopulmonary Resuscitation With Ventricular Unloading in Fulminant Influenza A (H1N1) Perimyocarditis: A Case Report. [Journal Article]Case Rep Crit Care. 2026; 2026:6129145.CR
- CONCLUSIONS: In fulminant perimyocarditis with refractory arrest, rapid activation of an extracorporeal cardiopulmonary resuscitation (ECPR) pathway with early VA-ECMO-combined with ventricular unloading-can bridge to myocardial recovery and favorable neurological outcome. Regional systems enabling safe transfer under continuous cardiopulmonary resuscitation (CPR) are critical.

- Iliohypogastric and Ilioinguinal Nerve Block for Awake ECPella Cannulation in Pediatric Patients With Nonpostcardiotomy Cardiogenic Shock. [Journal Article]ASAIO J. 2026 Oct 30. [Online ahead of print]AJ
- In pediatric cardiogenic shock, endotracheal intubation may carry a high risk of cardiac arrest during anesthesia induction. This single-center retrospective case series (2020-2025) evaluated the feasibility, safety, and outcomes of ultrasound (US)-guided ilioinguinal-iliohypogastric (IhIG) nerve blocks (0.05-0.1 ml/kg of 0.25% ropivacaine) under light sedation to facilitate awake venoarterial ex…

- American Society of Anesthesiologists Guidance on Periprocedural Fasting in Patients Receiving Tube Feeding. [Journal Article]
- In the absence of guidelines from professional societies, periprocedural fasting practices in enteral nutrition vary widely and often lead to preventable, clinically significant nutritional deficits. To address the heterogeneity of current clinical practice, the authors aimed to develop expert consensus on best practices for the periprocedural management of tube feeding using a modified Delphi co…

- TREM-1 in Sepsis and Septic Shock: From Pathophysiology to Precision Immunomodulation. [Journal Article]Semin Respir Crit Care Med. 2026 Oct 29. [Online ahead of print]SR
- Sepsis remains one of the leading causes of death worldwide, and decades of immunomodulatory trials targeting inflammation have failed to improve outcomes, largely because of the biological heterogeneity of the host response. Triggering receptor expressed on myeloid cells-1 (TREM-1) has emerged as a particularly relevant pathway in this context: rather than initiating pathogen recognition, TREM-1…

- Adjunctive therapeutic plasma exchange as rescue therapy in septic shock due to streptococcal infection: a multicentric retrospective cohort study. [Journal Article]
- CONCLUSIONS: In SRSS, adjunctive TPE was associated with rapid hemodynamic stabilization and improved lactate kinetics. Exploratory adjusted survival analyses support prospective evaluation of TPE in SRSS.

- Ventilator-associated pneumonia after traumatic brain injury: a timing-sensitive ecological framework of neuroimmune-pulmonary vulnerability. [Review]
- CONCLUSIONS: This model proposes a timing-sensitive ecological framework in which established mechanisms may converge early and with particular clinical intensity after severe TBI, creating a vulnerable host-microbe state that may lower the threshold for clinically apparent infection. Future studies should distinguish isolated TBI from polytrauma, compare TBI with other acute brain injuries and non-neurological ventilated populations, and incorporate dynamic immune, microbial, and trajectory-based outcomes.

- Percutaneous Echo-Guided Cervical Venoarterial Extracorporeal Membrane Oxygenation Cannulation by Intensivists in Neonates and Children. [Journal Article]ASAIO J. 2026 Oct 28. [Online ahead of print]AJ
- Cervical cannulation is the standard approach for venoarterial extracorporeal membrane oxygenation (VA-ECMO) in neonates and small children but is traditionally performed surgically. We report our initial experience with bedside percutaneous ultrasound- and echocardiography-guided cervical VA-ECMO cannulation performed by intensivists. Three pediatric patients underwent percutaneous right common …

- Viscoelastic Hemostatic Testing as a Diagnostic Tool for Trauma-Induced Coagulopathy with a Focus on Arabian Gulf Countries: A Narrative Review. [Review]
- Trauma-induced coagulopathy (TIC) at admission to the emergency department is associated with severe bleeding, massive transfusion, increased morbidity, and mortality. There are significant regional variations in the accessibility and acceptance of point-of-care technology, such as viscoelastic hemostatic testing (VET), for coagulation monitoring and bleeding management in the Arabian Gulf counci…

- Procalcitonin-Informed Decision-Making in Abdominal and Emergency Surgery: A WSES Position Paper. [Review]Antibiotics (Basel). 2026 Oct 21; 15(10).A
- Background: Procalcitonin (PCT) may support decision-making in abdominal and emergency surgery when infection is difficult to distinguish from non-infectious inflammation. Its interpretation remains challenging because clinical settings, sampling schedules, and proposed thresholds vary. This WSES position paper examines the evidence across six clinical scenarios to provide a practical framework f…

- Heart Under Pressure: From Scene to Scalpel in Cardiac Tamponade From a Left Ventricular Stab Injury. [Case Reports]Cureus. 2026 Sep; 18(9):e116967.C
- Penetrating cardiac injuries carry exceptionally high mortality, especially when the left ventricle is involved, and survival hinges on rapid diagnosis and immediate surgical intervention. We describe a case of penetrating cardiac trauma with acute tamponade, emphasizing the role of coordinated care from scene to operating theatre. A 30-year-old male presented with multiple precordial stab wounds…

- Physiology-Guided Management of Acute Right Ventricular Failure and Pulmonary Hypertension in Critically Ill Patients: Hemodynamic Phenotypes, Ventilator Interactions, and Rescue Strategies. [Review]
- Patients with acute right heart failure or pulmonary hypertension require cautious resuscitation because the right ventricle is highly preload-sensitive and has a narrow therapeutic window; both hypovolemia and volume overload may worsen hemodynamics. Echocardiographic guidance can help optimize fluid administration. The management of right heart failure also includes increasing contractility and…

- •Amniotic fluid embolism is a rare condition with high maternal mortality. It typically occurs during labor or delivery, and the pathogenesis has not yet been fully elucidated. •Survival of parturients with amniotic fluid embolism requires early recognition, rapid initiation of treatment, and access to skilled care based on the management of critically ill obstetric patients. •The classic clinica…

- CONCLUSIONS: TI-CA occurred in 1.2% of TIs performed in the PICUs. Odds of TI-CA increased incrementally with successive TI attempts. Video laryngoscopy was protective against TI-CA. Mediation analysis revealed that reduced TI attempts accounted for 13% of the protective benefit associated with video laryngoscopy.

- Organ damage after hemorrhagic shock: roles of anesthesia, shock duration, and assessment time. [Journal Article]An Acad Bras Cienc. 2026; 98(4):e20251220.AA
- Hemorrhagic shock (HS) is a condition associated with organ failure. Animal models are widely used to study pathophysiology and potential therapies. Our aim was to clarify how anesthetic choice, shock duration, decompensation phase length, and time post-reperfusion influence plasma damage markers in HS. For this, male rats were anesthetized with either thiopental or ketamine/xylazine and subjecte…
