- CONCLUSIONS: This extraordinarily rare case of sequential bilateral primary and secondary breast AS underscores the aggressive biological behavior of this malignancy and the limitations of current therapeutic strategies. It highlights the urgent need for identifying specific diagnostic biomarkers and novel therapeutic targets to enable early detection and improve the dismal prognosis associated with this disease.

- CONCLUSIONS: Tumor location influenced operative management and was associated with post-resection survival, but this was attenuated after excluding operative deaths. Among operative survivors, long-term outcomes were driven predominantly by histology, extracardiac disease, and gross residual disease. Although surgical resection remains a cornerstone of treatment, persistently poor survival highlights the need for effective biology-driven systemic therapies.

- CONCLUSIONS: In this single-institution cohort of pediatric and AYA patients, mastectomy was associated with worse observed OS. Nearly one-third of patients who received genetic testing had a pathogenic mutation. Recurrence was noted in over one-third of patients. These findings provide institutional-level outcome data and highlight the need for larger multi-institutional studies in this rare population.
