  • CONCLUSIONS: Oral ulcerative events are infrequently reported during TCZ therapy and may be underrecognized; however, the available evidence is heterogeneous and does not establish a uniform clinical phenotype or a definitive causal relationship. Pre-existing disease-related ulceration should be distinguished from newly occurring treatment-emergent events. Careful assessment of temporal relationships, underlying disease, concomitant therapies, and alternative etiologies is important when evaluating oral ulceration in patients receiving TCZ.

  • CONCLUSIONS: Parents of children, particularly mothers, frequently perceive their children as vulnerable. The positive correlation between maternal and paternal scores suggests shared psychosocial responses within the family environment. Assessment of parental vulnerability perceptions may contribute to family-centered care in pediatric FMF follow-up. Key Points • Mothers of children with FMF had higher vulnerability perception scores than fathers. • Coexisting PFAPA diagnosis was associated with higher paternal vulnerability scores. • M694V mutation and family history were not associated with parental vulnerability perception. • Psychosocial evaluation of parents may be important in the multidisciplinary follow-up of children with FMF.

  • CONCLUSIONS: CRP and SAA are reliable markers of PFAPA activity but lack discriminating value vs. infection. IFN-γ, particularly in combination with IL-6, is the most promising candidate for differentiation, though current evidence is limited and may be biased. No circulating biomarker distinguishes PFAPA from other autoinflammatory diseases, representing a major unmet need. PFAPA, therefore, remains a clinical diagnosis requiring exclusion of monogenic syndromes.
