- BackgroundThe optimal approach to fixation techniques in cartilage repair of the ankle remains unclear due to the predominance of low-quality evidence and limited high-level studies. To address this gap, an international group of experts convened to establish consensus-based recommendations grounded in the best available evidence, formulated during the 2017 International Congress on Cartilage Rep…

- Residual meniscal extrusion was not associated with 2-year clinical outcomes after posterior lateral meniscus root repair in a cohort predominantly undergoing anterior cruciate ligament reconstruction. [Journal Article]J ISAKOS. 2026 Sep 30; :101229. [Online ahead of print]JI
- CONCLUSIONS: In this small cohort, predominantly comprising patients treated with combined meniscal root repair and ACL reconstruction, Lysholm scores improved over 2 years. Within the limited observed extrusion range of 1.20 to 3.15 mm, no statistically significant association with clinical outcomes was demonstrated.

- Autograft Medial Patellofemoral Ligament Reconstruction Provides Comparable Patient-Reported Outcomes and Failure Rates With Fewer Complications Versus Allograft Reconstruction in Adolescent Patients: A Systematic Review. [Review]Arthroscopy. 2026 Sep 30. [Online ahead of print]A
- CONCLUSIONS: Both autograft and allograft MPFL reconstructions improve patient-reported outcome measures after patellar dislocation in adolescent patients. Autografts may provide less risk of complications with comparable rates of failure to allograft reconstructions in adolescent patients; however, the inherent heterogeneity of the present study limits any definitive conclusions.

- The Incidence and Breadth of Postoperative Pudendal Nerve Palsy and Its Relationship to Table Traction During Hip Arthroscopy With a Perineal Post. [Journal Article]Am J Sports Med. 2026 Sep 30; :3635465261485701. [Online ahead of print]AJ
- CONCLUSIONS: PNP symptoms were common after HA performed with perineal post traction, but most resolved within 6 months and all by 1 year. Increased traction duration and perineal pressure were significant predictors of symptom development, and persistent dysfunction correlated with lower hip-specific outcome scores.

- CONCLUSIONS: The Surgical Value Framework complements rather than replaces established patient-reported outcome measures and complication reporting. By matching outcome measurement to the claims made for a pathway, it provides a more balanced basis for clinical evaluation, research and service design.

- Low-Field Magnetic Resonance Imaging Features of Axial Proximal Sesamoid Osteomyelitis and Intersesamoidean Ligament Rupture in a Horse. [Case Reports]Vet Radiol Ultrasound. 2026 Nov; 67(6):e70242.VR
- A 7-year-old Warmblood mare used for show jumping was evaluated for progressive left hindlimb lameness of 5 weeks' duration, localized clinically to the metatarsophalangeal region. Radiography revealed ill-defined osteolysis of the axial apical regions of both proximal sesamoid bones (PSBs). Due to rapid clinical deterioration, low-field magnetic resonance imaging was performed and demonstrated a…

- Fabella Syndrome Following Arthroscopic Lateral Meniscectomy with Serial Imaging Findings: A Case Report. [Case Reports]JBJS Case Connect. 2026 Jul 01; 16(3).JC
- CONCLUSIONS: This case highlights a potential relationship between altered load distribution after meniscectomy and progressive fabellar hypertrophy. Fabella syndrome should be considered in patients with persistent posterolateral knee pain after meniscectomy.

- All-Inside Medial Meniscus Root Repair Improves Function With Results Comparable to Tibial Pullout Method, but Both Methods Fail to Correct Extrusion: A Systematic Review of Clinical and Biomechanical Studies. [Review]Arthroscopy. 2026 Sep 29. [Online ahead of print]A
- CONCLUSIONS: The all-inside repair, like the conventional pullout method, is a safe and effective method of medial meniscal root repair with reliable effects on joint's biomechanics, patients' function, minimal clinically important difference rates, and meniscus healing. Importantly, neither technique consistently restores meniscal position, because meaningful correction of extrusion remains unachieved. Given the substantial heterogeneity and predominantly nonrandomized nature of the available evidence, surgical decision-making should be individualized based on procedural context, concomitant procedures (e.g., osteotomy or additional tunnel requirements), and surgeon preference rather than presumed differences in efficacy.

- The Use of Unloader Bracing in the Management of Knee Osteoarthritis: A Modified Delphi Consensus Study. [Journal Article]Arthroscopy. 2026 Sep 29; :e70579. [Online ahead of print]A
- CONCLUSIONS: This modified Delphi consensus study provides expert-derived recommendations for the use of unloader bracing in the management of knee OA, across nonoperative, preoperative, perioperative, and postoperative contexts. Although unloader bracing should not replace definitive surgical options where indicated, it may serve as a valuable adjunct or alternative treatment in selected patients, particularly those with unicompartmental disease with correctable malalignment.

- Editorial Commentary: Suture Configuration Matters When Fixing Proximal Hamstring Tears. [Editorial]Arthroscopy. 2026 Sep 29. [Online ahead of print]A
- As endoscopic techniques for the management of proximal hamstring injuries have evolved, new challenges have emerged. Tendon-to-bone healing remains the ultimate goal, but a strong time-zero fixation construct is necessary to resist tension and facilitate healing. The constraints of an endoscopic environment limit the surgeon's ability to pass suture and create constructs that are relatively stra…

- Multicentre randomised controlled trial protocol in China comparing postless hip arthroscopy with conventional perineal post-assisted hip arthroscopy to reduce perineal complications in patients undergoing hip arthroscopy for femoroacetabular impingement: the POSTLESS-HIP trial. [Randomized Controlled Trial]BMJ Open. 2026 Sep 28; 16(9):e118402.BO
- Perineal complications-including nerve injury (eg, pudendal neurapraxia), soft tissue injury and sexual or urinary dysfunction-are well-documented adverse effects of conventional hip arthroscopy using a perineal post. Postless hip arthroscopy avoids direct perineal pressure, but high-quality randomised evidence is lacking.
