Prior authorization (PA) requirements imposed by Medicare Advantage (MA) and commercial insurers have evolved into a systematic barrier to guideline-indicated cardiac electrophysiology (EP) care, producing measurable patient harm, substantial administrative burden on physicians and allied professionals, and widening health care disparities. This paper examines the financial architecture of the MA capitation model and the structural incentives it creates for denial, characterizes the root causes of the denial system, including third-party denial organizations, artificial intelligence-based algorithms, and opaque coverage criteria; reviews federal oversight findings from the Office of Inspector General and the Senate Permanent Subcommittee on Investigations, and analyzes procedure-specific denial patterns for catheter ablation of atrial fibrillation, implantable cardioverter-defibrillator therapy, left atrial appendage occlusion, and cardiac resynchronization therapy with defibrillation. Federal audit data establish that a substantial proportion of denied PA requests meet Medicare's own coverage criteria and that the majority of appealed denials are overturned, yet most wrongful denials go uncontested. Clinical outcome data confirm that PA delays have direct clinical consequences across EP procedure categories. Acute appendicitis within a Spigelian hernia is a rare clinical entity that may present with atypical symptoms, resulting in diagnostic delay and complex operative decision-making. Previously reported cases have most commonly presented with localized right lower quadrant pain, abdominal wall masses, incarceration, bowel obstruction, or strangulation. Spigelian hernias occur along the semilunar line and may contain intra-abdominal structures, including the appendix, resulting in non-classic symptoms of appendicitis. We report a case of a 52-year-old woman with a history of cerebrovascular accident with residual left-sided weakness, prior laparotomy and colostomy following a gunshot wound to the left lower quadrant (2012), and atrial fibrillation with poor adherence to apixaban, who presented with progressive bilateral lower extremity pain and right lower quadrant pain radiating to the groin, with the atypical lower extremity symptoms initially prompting neurologic and musculoskeletal evaluation and delaying recognition of the underlying abdominal pathology. Initial vital signs were notable for mild tachycardia, and laboratory evaluation demonstrated mild leukocytosis. Over approximately one month, her progressive lower extremity pain was initially attributed to neurologic or musculoskeletal pathology, resulting in nondiagnostic spinal imaging before CT of the abdomen and pelvis established the diagnosis. CT imaging of the abdomen and pelvis ultimately revealed a right Spigelian hernia containing an inflamed appendix, consistent with acute appendicitis within the hernia sac. She underwent robotic appendectomy and hernia repair with mesh placement. Intraoperatively, a suppurative appendix within a 4-cm Spigelian hernia was confirmed. Her postoperative course was complicated by a superficial fluid collection managed with intravenous clindamycin and piperacillin-tazobactam for three days, with subsequent clinical improvement. This case is notable for the atypical presentation and resulting diagnostic delay. It highlights the importance of considering abdominal wall hernias and anatomic displacement of intra-abdominal structures in patients with persistent groin or lower quadrant pain, particularly when neurologic or musculoskeletal evaluation is unrevealing. Cross-sectional imaging remains essential for identifying uncommon hernia-related causes of appendicitis.

The primary intended audience for this guideline is clinicians who care for patients across the spectrum of cardiovascular-kidney-metabolic syndrome, an interrelated condition characterized by the interconnections among metabolic risk factors (including obesity and type 2 diabetes), chronic kidney disease, and cardiovascular disease.

An 85-year-old male presented to our emergency department with a three-day history of nausea, vomiting, diffuse lower abdominal pain, and diarrhea. His last bowel movement was in the morning prior to the evaluation. Physical exam demonstrated diffuse abdominal tenderness without peritoneal signs. CT scanning of the abdomen and pelvis (without contrast) was concerning for low-grade obstruction. He was observed for the next 24 hours. Although some improvement occurred, an elevated serum lactate and deteriorating renal function resulted in the decision to perform diagnostic laparoscopy to assess the presumed small bowel obstruction. Operative findings suggested the possibility of malrotation, so an open laparotomy was performed. This operation demonstrated the presence of Ladd's bands with torsion of the small bowel. The Ladd's bands were lysed, the detorsed small bowel was then placed on the right side of the abdomen, and the colon was placed on the left side with the cecum in the left upper quadrant, allowing broadening of the mesentery. The appendix was also removed in accordance with Ladd's procedure. The patient tolerated the operation well but required extended postoperative care to manage his renal failure and new-onset atrial fibrillation. The patient was discharged on the 10th postoperative day and followed for the next year and a half without subsequent problems.
