  • CONCLUSIONS: This case highlights that a normal PSA level does not exclude prostatic involvement by bladder-derived urothelial carcinoma. For patients with bladder neck or prostatic masses, especially those with normal PSA but imaging evidence of bladder-prostate junction involvement, an immunohistochemical panel including GATA3, CK7, CK20, p63, Uroplakin III, PSA, and NKX3.1 should be used early for differential diagnosis. Prostatic stromal invasion combined with lymphovascular invasion, perineural invasion, and a positive surgical margin may indicate highly aggressive disease and an increased risk of early distant metastasis, requiring intensified postoperative risk assessment, systemic treatment decision-making, and close follow-up.

  • CONCLUSIONS: This study has demonstrates that the KSTG Pill can significantly regulate M1 macrophage polarization to alleviate the prostatic hyperplasia and related hormonal level in the BPH rats. Its therapeutic mechanism is closely related to suppression of aberrant activation of the TLR4/MyD88/NF-κB signaling pathway in the prostate tissue, thereby reducing the polarization of macrophages to the M1 phenotype and the release of downstream pro-inflammatory factors. These findings provides experimental support for its clinical application, and and valuable insights for the development of novel BPH treatment strategies targeting the immune-inflammatory microenvironment.

  • CONCLUSIONS: Men undergoing benign prostatic hyperplasia procedures in the Veterans Affairs system present with progressively greater medical complexity, more strongly associated with year of surgery than patient age alone. Transurethral resection of the prostate remains predominant despite newer alternatives. Prostatic urethral lift had surpassed photoselective vaporization of the prostate as the second most popular option in recent years. However, transurethral resection of the prostate utilization has risen since 2021 with a corresponding decrease in prostate urethral lift.
