  • CONCLUSIONS: This case report demonstrates that achieving therapeutic ceftaroline concentrations for patients on ECMO may be feasible using current oxygenators. Clinicians could consider continuing ceftaroline therapy following ECMO cannulation for patients achieving good microbiologic response.

  • CONCLUSIONS: S. pneumoniae functions as an integrated adaptive system rather than a pathogen with independent resistance and evasion mechanisms. Iterative vaccine reformulation is approaching diminishing returns without parallel investment in serotype-independent protein vaccines and whole-genome sequencing surveillance. The mismatch between surveillance infrastructure in high-income countries and LMIC settings, where the greatest burden is concentrated, represents a structural inequity limiting global applicability of current policies. Priority areas include expansion of genomic surveillance to LMIC settings, clinical trials with mucosal protection endpoints for protein vaccine candidates, and explicit development of therapeutics with demonstrated cerebrospinal fluid penetration against multidrug-resistant pneumococci.

  • CONCLUSIONS: Antimicrobial sequestration within the ECMO circuit is detectable and drug- and component-specific. However, overall impact on systemic antimicrobial exposure appears limited. Inadequate PKPD target attainment in a subset of antimicrobial treatments likely reflects the marked PK variability of critical illness rather than ECMO-related drug loss alone. These exploratory results support the use of therapeutic drug monitoring to individualize antimicrobial therapy in ECMO-supported patients and the need to conduct further population PK studies to better characterize determinants of antimicrobial exposure and to clarify the clinical relevance of circuit-related drug sequestration.
