  • CONCLUSIONS: Cocaine-provoked PVT causing CAE in inherited thrombophilia is a rare mechanism requiring embolic source evaluation and tailored management.In young patients with cryptogenic CAE, cocaine use with inherited thrombophilia should prompt PVT evaluation. When post-percutaneous coronary intervention antiplatelet therapy requires anticoagulation, P2Y12 de-escalation to clopidogrel reduces bleeding risk.

Although PREVENT and FRS showed nearly similar classification of high risk, 80% of patients were still not identified as high risk before their event, underscoring the need for South Asia-specific risk score development and validation and alternative preventive strategies.

  • CONCLUSIONS: Spontaneous reperfusion at admission was observed in about 20% of STEMI patients who received a pre-hospital loading dose of antithrombotic therapy including P2Y12 inhibitors, and was associated with markers of decrease of infarct size. The significant increase of pre-admission cardiac arrests observed in the TIMI 3 group could be specifically explored as it might be a concern regarding future pre-hospital reperfusion therapy strategies.

  • CONCLUSIONS: In this post hoc analysis of the randomized ADAPT AF-DES trial, NOAC monotherapy beyond 12 months after PCI was associated with a substantial reduction in bleeding regardless of PCI complexity. Although limited by low absolute event rates, ischemic outcomes did not differ significantly between treatment strategies. These findings suggest that a bleeding-focused long-term antithrombotic strategy may be reasonable in patients with atrial fibrillation and chronic coronary syndrome, including those with prior complex PCI, although prospective validation remains warranted.

  • CONCLUSIONS: Among DM patients who completed the standard duration of DAPT after PCI, clopidogrel monotherapy was associated with a lower risk of a composite of death from any cause, myocardial infarction, and stroke compared with aspirin monotherapy, without increased rates of bleeding. There was no significant interaction between DM and antiplatelet monotherapy with respect to MACCE.   • CONCLUSIONS: Among patients with multivessel coronary artery disease who were in stable condition 12 months after implantation of a drug-eluting stent, extending DAPT with clopidogrel and aspirin for an additional 12 months led to a lower risk of death from cardiovascular causes, nonfatal myocardial infarction, or nonfatal stroke than continuing aspirin alone, without an increased risk of bleeding.
