- Impact of changing open-label drug prescription policies on consent rates in a neonatal trial: a quasinatural experiment. [Journal Article]Arch Dis Child Fetal Neonatal Ed. 2026 Aug 10. [Online ahead of print]AD
- CONCLUSIONS: Restricting open-label access to an off-label investigational drug can substantially increase parental consent and patient enrolment in neonatal RCTs. To promote both ethical recruitment and optimal enrolment, future trials should explicitly discuss equipoise in each contributing unit and align local clinical practices with the trial design prior to initiation.

- Pairing ampakine with brief hypoxia evokes phrenic neuroplasticity via a spinal NMDA receptor-mediated mechanism. [Journal Article]
- Brief hypoxic episodes drive neuroplasticity in animal models and humans. Pretreatment with an allosteric α-amino-3-hydroxy-5-methyl-4-isoxazolepropionic acid (AMPA) receptor (R) modulator ("ampakine") enables a single hypoxic exposure to induce sustained increases in phrenic motor activity ["phrenic motor facilitation" (pMF)]. Phrenic nerve activity was recorded in anesthetized rats to determine…

- The present and future of antiarrhythmic drugs. [Editorial]JRSM Cardiovasc Dis. 2026; 15:20480040261431981.JC
- The saga of antiarrhythmic drug (AAD) therapy reflects a field shaped by empiricism and burdened by challenges related to safety. The Singh-Vaughan-Williams classification was a timely necessity that eventually slowed pharmacological innovation by prioritizing electrophysiologic properties over arrhythmogenic mechanisms. The present, limited pharmacological armamentarium demands efforts to reasse…

- Doxapram Exposure Was Not Associated With Adverse Neurodevelopmental Outcomes in Very Low Birth Weight Infants: A Monocentric Retrospective Cohort Study. [Journal Article]Acta Paediatr. 2026 Sep; 115(9):1982-1990.AP
- CONCLUSIONS: Doxapram exposure was not associated with adverse neurodevelopmental outcomes in VLBWI within the investigated dosage range, although definitive conclusions on drug safety cannot be drawn from medium-sized cohorts and confirmation in prospective randomised studies is required.

- Dosing practices of caffeine therapy for apnoea of prematurity: a retrospective single-centre observational study. [Journal Article]
- CONCLUSIONS: Caffeine therapy in this unit showed marked variability in dosing, discontinuation and recommencement, highlighting the individualised nature of bedside decision-making, which may reflect clinical response to therapy.

- Pharmacological treatment for apnoea of prematurity-the need for an individualised approach. [Review]
- Apnoea-the cessation of breathing-is a common condition in preterm infants due to the immaturity of their lungs, airway and brainstem respiratory drivers. Consequently, many preterm infants will receive pharmacological interventions for the treatment of apnoea. Caffeine (or in some developing countries, aminophylline or theophylline) is usually given as first-line treatment, whereas doxapram is s…

- Comparative Effects of Aminophylline, Caffeine, and Doxapram in Hypoxic Neonatal Dogs Born by Cesarean Section. [Journal Article]
- CONCLUSIONS: Aminophylline, caffeine, and doxapram supported postnatal resuscitation in hypoxic neonatal puppies; however, caffeine demonstrated superior improvement in Apgar recovery, suggesting its potential as a preferred pharmacological adjunct in neonatal canine resuscitation protocols.

- Oral doxapram for apnea of prematurity: A randomized dosage trial. [Randomized Controlled Trial]
- CONCLUSIONS: Oral doxapram is feasible in very preterm infants with improvements in oxygenation and ventilation. Lower doses (12 mg/kg/6 h) are sufficient in most infants.

- Role of spontaneous effort during mechanical ventilation on lung injury in preterm lambs. [Journal Article]Am J Respir Crit Care Med. 2026 Mar 01; 212(3):511-522.AJ
- CONCLUSIONS: Different breathing efforts during PPV support of the preterm lung create complex lung states, each with unique and measurable injury events. This offers the potential to develop lung-protective strategies that target minimizing breathing-related injury.

- Doxapram May Improve Reliability of Oxygen Supplementation for Treatment of Anesthesia-Induced Hypoxemia of Moose (Alces alces). [Journal Article]J Wildl Dis. 2025 Oct 01; 61(4):1106-1110.JW
- Drug-induced hypoxemia can occur with the immobilization of free-living moose (Alces alces). We describe the physiologic response of 12 adult females immobilized with thiafentanil and xylazine and exhibiting clinical signs of hypoxemia before (pretreatment) and after (posttreatment) nasal oxygen supplementation (4 L/min) with or without intravenous doxapram.

- Management of Apnoea in Extremely Preterm Infants: A European Survey. [Journal Article]
- CONCLUSIONS: Despite consistency in the dosing and weaning of caffeine, there is much variation in the management of apnoea in preterm infants across Europe. Future research should focus on timing and dosage of caffeine, the use of doxapram, and strategies for optimising non-invasive respiratory support.

- KU124 (9,10,10-trioxo-N-(2-phenylphenyl)thioxanthene-3-carboxamide) as a novel inhibitor of TASK-1. [Journal Article]
- TASK-1 is a two-pore K[+] leak channel. The name, TASK-1, stands for TWIK-related acid-sensitive potassium channel 1, and this channel is encoded by the KCNK3 gene. TASK-1 channels are expressed in humans and modulate cell excitability in excitable cells such as neurons, cardiomyocytes, and vascular smooth muscle cells. TASK-1 inhibition is a mechanism of action for some respiratory stimulants, s…

- Evaluation of the safety of doxapram in premature neonates born before 28 weeks of gestation. [Multicenter Study]
- CONCLUSIONS: In children born before 28 weeks of gestation, doxapram treatment for apnea of prematurity was not associated with the occurrence of gastrointestinal events.

- Direct oral anticoagulants versus no anticoagulation for the prevention of stroke in survivors of intracerebral haemorrhage with atrial fibrillation (PRESTIGE-AF): a multicentre, open-label, randomised, phase 3 trial. [Randomized Controlled Trial]
- Direct oral anticoagulants (DOACs) reduce the rate of thromboembolism in patients with atrial fibrillation but the benefits and risks in survivors of intracerebral haemorrhage are uncertain. We aimed to determine whether DOACs reduce the risk of ischaemic stroke without substantially increasing the risk of recurrent intracerebral haemorrhage.

- Primary respiratory support of extremely preterm neonates in the Netherlands: a national survey. [Journal Article]Early Hum Dev. 2025 Feb; 201:106182.EH
- CONCLUSIONS: The study highlights diverse practices in primary neonatal respiratory support in the Netherlands, with significant variation in NIPPV settings while there is uniformity in CPAP use, underscoring the need for cohesive guidelines and training to standardize care and improve outcomes for extremely preterm neonates.
