- CONCLUSIONS: Our findings suggest that TFESI is an effective and safe method for alleviating pain and kinesiophobia in patients with chronic radicular pain due to LDH. Clinicians should be aware that a prolonged duration of symptoms is a risk factor that negatively influences the effectiveness of TFESI in cases of KP.

- CONCLUSIONS: Postoperative HDL-C levels were not significantly associated with 1-year postoperative functional recovery in the overall cohort of patients undergoing PLIF for LDH. In a pre-specified subgroup of patients without cerebral disease, postoperative HDL-C levels exhibited a significant U-shaped association with recovery (optimal range 1.1-1.8 mmol/L). These findings are hypothesis-generating and require validation in future studies.

- CONCLUSIONS: LDH location influences TFESI response, with mild compression and lateral herniations generally yielding favorable outcomes. However, technical proficiency and clinical predictors, such as positive Slump test, may be stronger determinants of success than herniation location alone. - CONCLUSIONS: The full-endoscopic monoportal Wiltse approach with inferior foraminal docking is a feasible, safe, and coherent technique for lateral lumbar disc pathology and foraminal stenosis. - CONCLUSIONS: Postoperatively, the patient showed early improvement in limb numbness and gait stability, and radiographs confirmed satisfactory implant position and cervical alignment. This case emphasizes that a "black disc" encountered during routine cervical discectomy should prompt immediate consideration of AKU/ochronosis, careful documentation and tissue sampling, and definitive metabolic testing (quantitative urinary homogentisic acid measurement, with HGD genetic testing when available). However, in the absence of biochemical or genetic confirmation, the intraoperative black-disc finding should be interpreted as a highly suggestive diagnostic clue rather than definitive proof of AKU. Standard cervical decompression and fusion principles remain effective for neurologic compromise, but recognition of a possible underlying metabolic disorder is essential for systemic evaluation and long-term surveillance.
