- CONCLUSIONS: BST using propofol and dexmedetomidine may warrant further investigation as an adjunct to multimodal management in patients with chronic insomnia, long-term alcohol use as a sleep aid, and withdrawal-related symptoms. However, improvements cannot be attributed specifically to BST, and its contribution and durability require prospective evaluation.

- CONCLUSIONS: The addition of bupivacaine to IT morphine is associated with a safety profile similar to morphine monotherapy, although with a significantly smaller frequency of cognitive impairment, and a trend toward fewer granulomas, despite higher median morphine doses. These real-world findings are consistent with the safety profile of this consensus-backed off-label practice.

- CONCLUSIONS: This study found a relatively high rate of diabetes technology adoption at our center compared to other Asian countries, highlighting its potential to reduce the risk of hypoglycemia and improve glycemic stability. However, technology users also had higher HbA1c levels, suggesting that diabetes technology is often prescribed to high-risk patients with poor glycemic control. Future research should focus on longitudinal studies to better understand and address barriers to optimal glycemic outcomes among diabetes technology users.

- CONCLUSIONS: To our knowledge, PROTECT-CS is the first adequately powered, multicenter, double-blind randomized trial designed to evaluate whether perioperative rhBNP prevents AKI after high-risk cardiac surgery. The study is intended to provide high-quality evidence to inform clinical practice and future guideline recommendations on pharmacological renal protection in cardiac surgery patients.
