- Liver sinusoidal endothelial cells phenotypes as early predictors of sepsis-associated liver dysfunction. [Journal Article]Life Sci. 2026 Sep 28; :124715. [Online ahead of print]LS
- CONCLUSIONS: Circulating endothelial phenotyping identifies a specific endothelial phenotype associated with subsequent SALD beyond total endothelial cell abundance and conventional bilirubin measurements. The HLA-DR ratio may provide complementary information for early risk stratification of SALD.

- Emergency Surgery for Diverticulitis: Does the Number of Days From Admission to Surgery Matter? [Journal Article]J Surg Res. 2026 Sep 28; 327:328-335. [Online ahead of print]JS
- CONCLUSIONS: Delayed emergency surgery for diverticulitis was associated with higher rates of MIS and/or primary anastomosis and increased 30-d postoperative morbidity and mortality.

- Response to Dobutamine in Septic Shock With Myocardial Dysfunction: A Single-Center Retrospective Study. [Journal Article]Crit Care Explor. 2026 Oct 01; 8(10):e1487.CC
- CONCLUSIONS: In septic shock with myocardial dysfunction, early hemodynamic responsiveness to dobutamine identified a subgroup with a more favorable clinical trajectory. Resting echocardiographic parameters did not identify responders. These findings are hypothesis-generating and require prospective confirmation.

- Personalized fluid resuscitation may reduce mortality in septic shock as compared with fixed-volume approach - a systematic review and meta-analysis of randomized controlled trials. [Journal Article]Intensive Care Med Exp. 2026 Sep 28; 14(1).IC
- CONCLUSIONS: Personalized fluid resuscitation, compared with fixed-volume resuscitation, indicated reduced 30-day mortality in septic patients. However, the observed mortality benefit should be interpreted cautiously given the relatively small overall sample size, predominance of small single-center studies, and moderate-to-low certainty of evidence.

- Mixed Invasive Aspergillus-Mucorales Pulmonary Coinfection in a Diabetic Patient: An Uncommon Dual Fungal Pneumonia and Literature Review. [Journal Article]
- Mixed pulmonary infection by Aspergillus and Mucorales is a rare condition but highly fatal in immunocompromised hosts. We report a 61-year-old diabetic man with diabetic kidney disease who presented with fever and cough. Bronchoscopy-guided lung biopsy and lavage MUCOR 28S PCR assay confirmed dual invasive Aspergillus and Mucorales coinfection. He developed massive hemoptysis, septic shock, and …

- Recurrent Euglycemic Diabetic Ketoacidosis Weeks After SGLT2 Inhibitor Cessation: A Case Report. [Journal Article]
- Euglycemic diabetic ketoacidosis (EDKA) is a complication of sodium-glucose cotransporter-2 (SGLT2) inhibitors that is being increasingly documented. It is challenging to diagnose, especially in critically ill patients because hyperglycemia is absent or mild. We report the case of a 71-year-old man with type 2 diabetes mellitus previously treated with dapagliflozin, which had been discontinued 2 …

- Surgical Management of Intestinal Obstruction: A Prospective Observational Study. [Journal Article]
- CONCLUSIONS: Intestinal obstruction at a tertiary care center presents with a bimodal etiology comprising adhesion-related SBO and malignancy-driven LBO. A structured protocol incorporating early CECT imaging, risk stratification by comorbidity indices, selective conservative management for SBO, and individualized surgical decision-making based on radiology and intraoperative findings results in acceptable outcomes. An overall morbidity of 41.5% and a 30-day mortality of 11% are consistent with established benchmarks for emergency bowel surgery in a tertiary care setting. Stoma formation remains an important and frequently utilized surgical strategy, particularly in the setting of malignant LBO and complicated SBO with bowel gangrene or perforation.

- Early versus late therapeutic plasma exchange in sepsis and septic shock: A retrospective observational study of 28-day mortality and biomarker trajectories. [Journal Article]Medicine (Baltimore). 2026 Sep 25; 105(39):e50831.M
- Sepsis involves dynamic inflammatory and endothelial responses, and the timing of extracorporeal therapies such as therapeutic plasma exchange (TPE) may influence clinical outcomes. However, the optimal timing of TPE remains unclear. This retrospective single-center observational study included adult patients with sepsis or septic shock who underwent TPE in the intensive care unit between January…

- Streptococcus suis (S. suis) is a zoonotic pathogen that causes severe diseases including meningitis, septic shock, and endocarditis in both pigs and humans. In swine, S. suis is one of the most important bacterial pathogens causing invasive diseases such as meningitis, septicemia, arthritis, and sudden death, with serotypes 1, 2, 7, and 9 being among the most frequently recovered from clinical c…

- Cross-Cohort Integration of Blood DNA Methylation and Sepsis Transcriptomes Prioritizes TP53INP1 as a Candidate Associated with B-Cell Transcriptomic Patterns in Pediatric HAdV-7-Associated Sepsis. [Journal Article]
- Human adenovirus type 7 (HAdV-7) can cause severe pneumonia and sepsis in children, but the associated host epigenetic and immune alterations remain poorly defined. We profiled peripheral-blood DNA methylation in 11 children with HAdV-7-associated sepsis (6 survivors and 5 non-survivors) and 5 pediatric controls without clinically detectable HAdV-7 infection using reduced representation bisulfite…

- Key Inflammatory Pathways, Biomarkers, and Targeted Management Strategies in Primary Total Joint Arthroplasty: A Narrative Review. [Review]
- Total joint arthroplasty is a surgical procedure with rising global incidence. Although a strong postoperative inflammatory response is necessary for tissue repair following primary arthroplasty, it may prove to be harmful if excessive or prolonged. This could compromise osseointegration, increase pain, and delay the detection of periprosthetic joint infection. This narrative review examines the …
