- CONCLUSIONS: Cytologic evaluation of OCP was limited by wide interobserver variability and inconsistent associations with outcomes. Agreement of CPs on presence of specific cytologic features did not enhance prediction of neoplasm or malignancy. Molecular testing provided more objective performance displaying high sensitivity; however, specificity was low and FNR was elevated. Clinicians should be made aware of the greater limitations associated with cytology and molecular test results of OCP compared to those reported for their nononcocytic counterparts.

- CONCLUSIONS: In this retrospective single-center, pathology-enriched cohort, the effect of AI-assisted C-TIRADS adjustment appeared to vary by sonographer experience. Mandatory adjustment may be more helpful for junior sonographers, whereas flexible integration may be more useful for senior sonographers. These findings should be interpreted as preliminary workflow observations and require validation in larger prospective multicenter studies.

- CONCLUSIONS: AI systems achieve high diagnostic accuracy for thyroid nodule malignancy, demonstrating performance comparable or superior to radiologists and risk stratification frameworks. As adjunctive tools, they safely reduce unnecessary biopsies and improve diagnostic consistency. Future clinical integration must prioritize multimodal data and radiomics to maximize utility.
