- CONCLUSIONS: This survey study of infection-prevention practice use found that US hospitals substantially expanded use of many CAUTI prevention practices, particularly surveillance and monitoring systems, over the past 20 years. However, variability in use of high-yield practices, such as reminders and nurse-driven discontinuation, persisted, underscoring the need for sustained, system-level strategies to optimize urinary catheter safety across health care settings.

- CONCLUSIONS: ED clinicians meaningfully increased their adherence to guideline-directed management of UTIs when publicly compared among their peers, safely reducing both the spectrum of prescribed antibiotics and DOT; increasing the availability of FSGs did not similarly increase guideline adherence. Sequentially phased peer-comparison feedback may serve as a scalable antimicrobial stewardship tool across the Military Health System and deployed environments.

- CONCLUSIONS: IVT vancomycin at 5 mg every 48 h via an Ommaya reservoir may represent a safe and effective salvage treatment for CoNS ventriculitis in preterm infants when systemic antibiotics fail. Therapeutic drug monitoring (TDM) may be particularly important because CSF vancomycin levels vary greatly between individuals. This case provides one of the first serial CSF vancomycin trough measurements in a very preterm infant, suggesting a potential empiric starting approach for IVT vancomycin in this population.
