- CONCLUSIONS: Intrawound administration of one gram of vancomycin powder in primary TKA resulted in minimal systemic exposure and showed no clinically meaningful nephrotoxicity signal. However, it was not associated with a statistically significant reduction in infection or wound-complication outcomes; therefore, these data do not support its routine prophylactic use in primary TKA.

- CONCLUSIONS: In patients with DRESS complicating progressive infective endocarditis, corticosteroids may adversely affect prognosis; timely removal of vegetations may effectively control the disease. In infective endocarditis with new onset intracranial hemorrhage, multidisciplinary discussion and appropriate surgical timing are critical to treatment.Multidisciplinary risk-stratified immunomodulation combined with surgical intervention can achieve favorable outcomes in this complex triad.

- Impact of surface structuring on antibiotic elution from bone cement in two-stage revision for periprosthetic joint infection. [Journal Article]
- CONCLUSIONS: Surface modification of PMMA cement influenced antibiotic elution in this in vitro experimental model, with increased surface depth resulting in enhanced vancomycin release. Gentamicin elution was less affected by surface geometry and showed significant improvement only with the greatest surface enlargement. These findings indicate that structured cement surfaces may influence local antibiotic release characteristics under controlled laboratory conditions. However, the clinical relevance of these surface modifications remains to be determined, as this study was limited to cement blocks incubated in phosphate-buffered saline and did not include a spacer, infection, or in vivo model. Further experimental and in vivo investigations are required to assess whether surface structuring can improve antibiotic delivery in clinically relevant settings.

- Acute bacterial endophthalmitis with focal vitritis at a nasal chandelier illumination port after scleral buckling. [Journal Article]
- CONCLUSIONS: Whereas previously reported cases of CSB-related endophthalmitis typically presented with widespread intraocular inflammation obscuring the primary infectious focus, this case captures the early focal progression of vitritis in a chronological sequence. The localization at the nasal chandelier port provides visual clues to a possible entry route. The appearance of a localized vitreous opacity corresponding to a chandelier insertion site-even if subtle-should raise suspicion of acute endophthalmitis and prompt timely evaluation and treatment. The route and timing of bacterial entry cannot be established from this case alone.
