- CONCLUSIONS: This case underscores that invasive aspergillosis should be considered in the differential diagnosis of chronic or subacute spondylodiscitis, even in apparently immunocompetent patients. Early diagnosis requires vertebral biopsy with dedicated fungal histopathology, mycological culture, species identification, and antifungal susceptibility testing to enable timely targeted therapy and early multidisciplinary management.

- CONCLUSIONS: Approximately 15 cases of Scedosporium apiospermum spondylodiscitis have been reported, with a minority occurring in immunocompetent patients following near-drowning. This case highlights that pathogens identified after submersion should not be dismissed as colonization, as delayed invasive infection may occur even in immunocompetent individuals.

- CONCLUSIONS: Overall, this analysis demonstrates the capacity of PBPK modeling to predict DDIs across different genotypes and dosing scenarios. Although the relative increase in steady-state VCZ exposure was predicted to be similar across genotypes, this analysis demonstrated that CYP2C19 intermediate and poor metabolizers are at elevated risk of VCZ toxicity in the presence of strong CYP3A4 inhibition.

- Antifungal prophylaxis in patients with haematological malignancies receiving chemotherapy or haematopoietic stem cell transplantation: a systematic review and network meta-analysis. [Review]
- CONCLUSIONS: No regimen was consistently superior across settings and outcomes. Voriconazole and liposomal amphotericin B had the clearest evidence versus inactive control. Posaconazole effects were comparator-dependent; peri-HSCT itraconazole showed fewer IFDs but more discontinuations. No mortality benefit was established. Prophylaxis should be risk-adapted to clinical setting, baseline mould risk, formulation, tolerability, and feasibility.

- CONCLUSIONS: This is the first study to identify an association between the Erg3 T208I substitution and putative cross-resistance to fluconazole and voriconazole in T. marneffei. The findings of expand the known genetic mechanisms underlying putative antifungal resistance in T. marneffei.

- CONCLUSIONS: Acanthamoeba and Fusarium coinfection can cause infectious crystalline keratopathy. Treatment was difficult due to the crystalline keratopathy itself and the concurrent coinfection. To prevent recurrent progression of infectious crystalline keratopathy caused by Acanthamoeba and Fusarium coinfection, vigilant follow-up with long-term aggressive medical treatment may be necessary, even when an early clinical improvement is observed.

- CONCLUSIONS: Persistent infiltrates with high IgE and Candida isolation should suggest ABPM, not intensified antibiotics. ABPC can occur in older patients without asthma.
