Orbital conjunctival epithelial inclusion cysts are a rare complication of strabismus surgery. A subconjunctival cystic mass with translucent fluid is the most common presentation. The authors describe a case of an epithelial inclusion cyst presenting as an occult orbital mass during reoperation strabismus surgery almost 70 years after strabismus surgery. PURPOSE:

To determine the preoperative factors affecting the efficacy in bilateral lateral rectus recession surgery for infantile exotropia.

METHODS:

This was a retrospective study of 50 patients with infantile exotropia who had bilateral lateral rectus recession surgery correction with more than 1 year of follow-up. Preoperative parameters influencing the response of the surgery were obtained and evaluated using multiple regression analysis including age at onset, age at surgery, interval between onset and surgery, preoperative deviation, refractive error, anisometropia, amount of surgery performed, presence of A- or V-pattern, and preoperative phase of exotropia. The response to surgery was defined as the change in deviation divided by the amount of the surgery.

RESULTS:

Thirty-two (64%) and 24 (48%) patients had successful outcome at 6 weeks and 1 year postoperatively, respectively. The mean response to surgery was 1.79 ± 0.63 prism diopters (PD)/mm recession at 6 weeks and 1.58 ± 0.75 PD/mm recession at 1 year postoperatively. Exotropic drift was 5.12 ± 7.27 PD at 6 weeks and 8.02 ± 6.81 PD at 1 year postoperatively. Pre-operative deviation was correlated with the response of surgery at 6 weeks (β = 0.342, P = .020) and 1 year (β = 0.391, P = .007) postoperatively.

CONCLUSIONS:

Surgical response decreases over time due to the exotropic drift. Larger preoperative distance deviation was associated with larger surgical response. Both the exotropic drift and the preoperative deviation should be considered in applying the surgical dose in infantile exotropia.[J Pediatr Ophthalmol Strabismus 20XX;XX(X):XX-XX.].

PURPOSE:

To evaluate the possible risk factors in patients with consecutive exotropia following esotropia surgery.

METHODS:

Medical records of patients who had comitant esotropia surgery between June 1999 and April 2011 were reviewed. Those who developed consecutive exotropia composed the exotropia group; patients matched for age and duration of follow-up who did not develop consecutive exotropia composed the no exotropia group. The charts of the patients were reviewed and possible risk factors for development of consecutive exotropia were investigated.

RESULTS:

The average ages of 47 patients in the exotropia group and 54 patients in the no exotropia group were 10.8 ± 8.7 years (range: 1 to 41 years) and 8.5 ± 6.3 years (range: 1 to 30 years), respectively (P = .292). Amblyopia was detected in 31 (66%) and 12 (22.2%) patients in the exotropia and no exotropia groups, respectively (P = .004). Anisometropia was observed in 20 patients (42.6%) in the exotropia group and 5 patients (9.3%) in the no exotropia group (P = .003). Preoperative average esodeviation values were 42.5 ± 8.3 prism diopters (PD) (range: 25 to 60 PD) in the exotropia group and 42 ± 9.4 PD (range: 20 to 65 PD) in the no exotropia group (P = .673). Postoperative deviations were 32.8 ± 23 PD exotropia (range: 10 to 90 PD exotropia) in the exotropia group and 4.4 ± 4.2 PD esotropia (range: 0 to 10 PD esotropia) in the no exotropia group (P = .000). Asymmetric surgery had been performed in 61.7% of the exotropia group (n = 29) and 9.3% of the no exotropia group (n = 5) (P = .000). Limitation of adduction was detected in 14 patients (29.8%) in the exotropia group; none was noted in the no exotropia group (P = .000). The mean interval between the initial surgery and the onset of consecutive exotropia was 11.1 ± 15.1 months (range: 0 to 126 months).

CONCLUSIONS:

Anisometropia, amblyopia, asymmetric surgery, and postoperative adduction deficit were associated with the development of consecutive exotropia. Long-term follow-up should be considered because consecutive exotropia can develop after months or years.[J Pediatr Ophthalmol Strabismus 20XX;XX(X):XX-XX.].

Malignant extrarenal rhabdoid tumor is a rare and highly aggressive tumor of childhood. Intraocular involvement by malignant extrarenal rhabdoid tumor has only been described once as a metastasis. No report exists in published literature describing a primary intraocular malignant extrarenal rhabdoid tumor. The authors report the first such case along with its clinico-radiological features and histopathologic and electron microscopic characteristics.

PURPOSE:

To describe a cohort of children with late-recognized primary congenital glaucoma (LRPCG), including age of presentation, age-related diagnostic signs, clinical abnormalities, and results of glaucoma surgery.

METHODS:

The medical records of 31 patients (49 eyes) with PCG recognized after 1 year of age were reviewed retrospectively. Patients were confirmed to have PCG based on their increased intraocular pressure (IOP), anterior segment abnormalities including findings on gonioscopy, and the absence of other causes of childhood glaucoma. The outcome of glaucoma surgery was reviewed and success measured by assessment of the relative control of IOP, occurrence of significant complications, and need for additional glaucoma surgery.

RESULTS:

Average age at diagnosis of glaucoma was 4.7 years (36% diagnosed at > 4 years of age). The most common initial diagnostic signs were corneal enlargement (46%, average age of 2.0 years), photophobia (20%, average age of 3.3 years), and suspected poor visual acuity (32%, average age of 9.9 years). Corneal cloudiness was not an initial sign for any patient. Haab's striae were present in 60% of the affected 49 eyes. Gonioscopy findings were abnormal in 82%, but the ciliary body band was seen in 81% and the scleral spur was visible in 47%. Sixty-one goniotomy procedures were performed for 39 eyes with overall success in 95% (37 eyes) and complete success in 65% (27 eyes). The final visual acuity was 20/200 or worse in 31% (15 eyes) and 20/40 or better in 60% (29 eyes).

CONCLUSION:

An awareness of and familiarity with the subtle diagnostic signs of LRPCG can enable its differentiation from primary juvenile glaucoma and contribute to earlier recognition and treatment. Glaucoma surgery is often required for LRPCG and goniosurgery is the recommended initial procedure.[J Pediatric Ophthalmol Strabismus 2013;50:XX-XX.].

PURPOSE:

To determine the incidence of congenital nasolacrimal duct obstruction (CNLDO) in premature and full-term children.

METHODS:

Medical records of 200 premature and 200 full-term children from July 2004 to November 2009 at the Peri-Peri Health Center were retrospectively reviewed. Fifty-three nasolacrimal ducts in 32 premature children (21 patients were bilateral) and 9 nasolacrimal ducts in 7 full-term children (2 patients were bilateral) diagnosed as having CNLDO were retrospectively examined.

RESULTS:

Of the 200 premature children, 32 (16%) had CNLDO compared with 7 (3.5%) of the 200 full-term infants (P < .0001). Thirty premature infants and five full-term children underwent conservative treatment. Bilateral obstruction was found in all 3 extremely premature children (gestational age < 28 weeks), in 9 (69.23%) moderately premature children (gestational age of 28 to 31 weeks), in 9 (56.25%) mildly premature children (gestational age of 32 to 36 weeks), and in 2 (28.57%) full-term children (gestational age > 37 weeks).

CONCLUSIONS:

This study reported a higher incidence of CNLDO in preterm infants when compared to full-term infants. This might be expected in light of the normal development of the nasolacrimal duct system, the patency of which normally occurs after the preterm infant is born.[J Pediatr Ophthalmol Strabismus 20XX;XX(X):XX-XX.].

PURPOSE:

New objective photoscreeners are emerging with encouraging national guidelines and pediatric providers can be reimbursed for photoscreening.

METHODS:

Nine objective screeners were applied to two emmetropic subjects with or without known power contact lenses to induce spherical and cylindrical anisometropia close to American Association for Pediatric Ophthalmology and Strabismus (AAPOS) thresholds.

RESULTS:

The screeners produced near linear pupil crescents and estimated refractions for induced anisometropia. Most performed well with AAPOS 2003 validation; however, the iPhone (Apple, Cupertino, CA) was too sensitive, whereas the remote autorefractors using Vision in Preschoolers Study criteria were less sensitive.

CONCLUSIONS:

The new, interpreted objective screeners appear valid for detection of anismetropia.[J Pediatr Ophthalmol Strabismus 20xx;xx:xx-xx.].

PURPOSE:

To report the outcomes of bilateral pediatric cataract surgery from eastern Nepal and northern India.

METHODS:

Preoperative, intraoperative, and postoperative data of 390 children who underwent surgery bilaterally between 2007 and 2009 were analyzed.

RESULTS:

Forty-two (10.8%) children came from Nepal and 348 (89.2%) from India (mainly Bihar State). Intraocular lens (IOL) implantation with posterior capsule opening and anterior vitrectomy were achieved in 386 (99.0%) children bilaterally. Median age at surgery was 7 years and 69.2% were male. At first presentation, 243 (62.3%) of the children were blind (< 3/60 in the better eye). After more than 1 year, 53.5% had a normal visual status (range: 6/6 to 6/18), 5.6% of children were still blind, and mean refractive error spherical equivalent was +1.0 ± 2.4 diopters. Astigmatism changed from suture-induced with the rule at discharge to against the rule within 3 weeks of surgery. Mean long-term astigmatic error was 1.0 ± 0.9 diopters after 1 year. Glaucoma was rare.

CONCLUSIONS:

Even in a setting with limited resources, successful, cost-effective, high-volume surgery for pediatric cataract is possible. Despite late presentation and limited follow-up, more than half achieved good outcomes after more than 1 year. Only 5.6% remained blind due to amblyopia or eye anomalies. Bilateral surgery during one hospital stay, IOL implantation with undercorrection according to age, aggressive surgery to prevent secondary cataract, intensive anti-inflammatory therapy, and provision of durable, high-quality spectacles to take home all proved beneficial because many children cannot attend for regular follow-up.[J Pediatr Ophthalmol Strabismus 20XX;XX(X):XX-XX.].
