  • CONCLUSIONS: EMS clinicians documented similar primary impressions and administered many of the same treatments across both upper and lower airway groups, demonstrating minimal differentiation in prehospital management of young children. Therefore, EMS impressions and treatments did not consistently align with the underlying etiology diagnosed at the ED. Addressing these gaps through protocol revisions, system-wide improvements, and targeted education may help strengthen the quality and safety of prehospital pediatric respiratory care.

  • CONCLUSIONS: Although WALANT is recognized as a very safe procedure, ischemic complications can occur in hand surgery in rare cases. The present study shows an insufficient level of knowledge regarding the antidote phentolamine in the treatment of ischemic complications associated with the WALANT technique. Users of the WALANT technique must be familiar with the procedure and its potential risks in order to be able to respond appropriately in an emergency. Even rare complications may, in case of doubt, lead to functional impairment and medikolegal consequences, particularly when phentolamine is not available. We therefore recommend always maintaining a stock of phentolamine and providing regular training sessions to ensure optimal patient safety.
