- CONCLUSIONS: Self-reported OTC safety behaviours should not be assumed to represent successful application of the underlying pharmacological reasoning. Correspondence was domain-dependent and appeared less consistent for risks requiring identification and interpretation of pharmacological exposures than for persistent symptoms requiring evaluation. These findings support making the pharmacological reasoning underlying OTC safety recommendations more explicit and informed the proposal of a clinical-pharmacology framework for future OTC safety communication.

- CONCLUSIONS: Cocaine-positive UDS results may substantially alter chronic opioid prescribing. Management may include increased monitoring, gradual discontinuation of full agonist opioids, or consideration of buprenorphine-based therapy in appropriately selected patients. These cases emphasize the importance of individualized risk assessment rather than a uniform response to an unexpected UDS result.

- CONCLUSIONS: In this NMA, OTC therapies showed meaningful efficacy and favorable safety for acute episodic migraine versus placebo, with ibuprofen and AAC demonstrating consistent benefits across 2 h pain and secondary outcomes. These findings support their use as effective, accessible options alongside newer oral agents. Larger trials and patient-level analyses are needed to refine efficacy, optimize dosing, and personalize migraine management.
