- The Evolving Landscape of Reduced-Intensity Conditioning: Balancing Efficacy and Toxicity Across Hematologic Malignancies. [Review]
- Allogeneic hematopoietic stem cell transplantation (allo-HSCT) has evolved from traditional myeloablative conditioning toward reduced-intensity conditioning (RIC) and strategies using donor-derived immune effector cells. This review summarizes trends in RIC, proposes a practical classification of regimens, and underscores personalized risk stratification and disease-specific conditioning.

- Fludarabine-treosulfan compared with fludarabine-melphalan conditioning for adult acute lymphoblastic leukemia. [Journal Article]Blood Adv. 2026 Oct 28. [Online ahead of print]BA
- The optimal conditioning strategy for adult acute lymphoblastic leukemia (ALL) undergoing allogeneic hematopoietic stem cell transplantation (allo-HSCT) remains undefined, particularly in older or medically vulnerable patients. Reduced-intensity regimens lower non-relapse mortality (NRM) but increase relapse risk. Treosulfan-based conditioning may represent an intermediate-intensity alternative, …

- Comparison of Fludarabine/Treosulfan Conditioning with Melphalan-Based Sequential Conditioning in Patients with MDS and Increased Blasts Undergoing Upfront Transplantation. [Journal Article]Cancers (Basel). 2026 Oct 14; 18(20).C
- CONCLUSIONS: This pilot series shows that upfront transplantation using Flu/Treo conditioning in high-risk MDS patients facilitates promising post-transplant outcomes and may serve as a backbone to further improve these results through innovative peri-transplant interventions.

- Comparative prognostic analysis of allogeneic hematopoietic stem cell transplantation regimens in the treatment of severe aplastic anemia. [Journal Article]
- CONCLUSIONS: (1)MSD-HSCT and Haplo-HSCT yield comparable 5-year OS. Haploidentical related donors serve as a reliable alternative when fully matched donors are unavailable. (2) No statistically significant difference in 5-year overall survival (OS) was observed among the CA, FCA, and FMA conditioning regimens. (3) the FMA regimen may be considered as a favorable alternative to avoid the cyclophosphamide related toxicities.

- Anthracycline-Free FLA-VEN with or Without Midostaurin in Newly Diagnosed Intermediate- and Adverse-Risk AML: A Single-Center Retrospective Study. [Journal Article]
- CONCLUSIONS: In this small exploratory cohort, anthracycline-free and G-CSF-free FLA-VEN, with or without midostaurin, was associated with high remission rates, no early mortality, and frequent bridging to allo-HSCT. These findings support prospective controlled evaluation of this anthracycline-sparing strategy.

- Successful First-Line Haploidentical Hematopoietic Stem Cell Transplantation in an Elderly Patient With Severe Aplastic Anemia and Complex Comorbidities: A Case Report. [Journal Article]Transplant Proc. 2026 Oct 15. [Online ahead of print]TP
- Standard management guidelines generally recommend immunosuppressive therapy (IST) as first-line treatment for older patients with severe aplastic anemia (SAA), whereas the role of upfront haploidentical hematopoietic stem cell transplantation (haplo-HSCT) in this population remains uncertain, particularly in patients with complex comorbidities. We report a 62-year-old woman with SAA, a history o…

- CONCLUSIONS: Condensed Bu/Mel/Flu before ex vivo CD34+-selected allo-HCT was feasible and met the prespecified safety endpoint in this pilot study primarily conducted in patients with myeloid malignancies. The condensed 5-day regimen was associated with a comparable toxicity profile to the prior sequential 8-day regimen and with favorable survival outcomes supporting further study of this MAC regimen in the context of ex vivo CD34+ selection.

- Allogeneic hematopoietic stem cell transplantation (HSCT) is a potentially curative therapy for malignant and non-malignant diseases; however, primary graft failure (GF) is a life-threatening complication, particularly with the increasing use of alternative donors and reduced-intensity conditioning (RIC) strategies. Data on outcomes and optimal management-especially in the pediatric population-re…

- Allogeneic stem cell transplantation in aggressive Adult T cell leukemia/lymphoma: a prospective multicenter study. [Journal Article]Blood Adv. 2026 Oct 07. [Online ahead of print]BA
- Adult T-cell leukemia/lymphoma (ATL) is associated with a dismal prognosis in aggressive subtypes, with a median overall survival (OS) of less than one year. In Japan, first line chemotherapy followed by allogeneic stem cell transplantation (allo-SCT) is recommended for eligible patients, yet this strategy is scarcely reported elsewhere, especially among ethnic minorities. We prospectively evalua…
