Diabetes mellitus, especially Type 2 Diabetes (T2DM), appears to be a worldwide health challenge marked by progressing hyperglycemia and related complications. Postprandial Hyperglycemia (PPHG) is a critical therapeutic target due to its strong link with cardiovascular risks. Inhibition of carbohydrate-hydrolysing enzymes, α-glucosidase and α-amylase, is an established strategy to mitigate PPHG. Current Inhibitors, including acarbose, miglitol, and voglibose, are effective but limited by gastrointestinal side effects. Isatin (1H-indole-2,3-dione) is a privileged scaffold in medicinal chemistry with diverse biological activities and modifiable sites (N-1, C-3, C-5), enabling optimisation for enzyme inhibition. A comprehensive literature search was conducted up to December 2025 using databases including Google Scholar, Web of Science, PubMed, traditional integrated knowledge resources, local dissertations, and books. The search employed keywords such as "α- glucosidase," "type 2 diabetes," "isatin scaffolds," "standard α-glucosidase enzyme inhibitors," and "isatin derivatives showing α-glucosidase and α-amylase inhibitory activity." This review compiles recent advances in isatin derivatives as potential α-glucosidase and α-amylase inhibitors, highlighting structure-activity relationships, synthetic strategies, including green chemistry and microwaveassisted synthesis, and dual inhibitory activity for improved efficacy and safety. Reported derivatives demonstrate remarkable potency, with some surpassing standard drugs in IC50 values and docking scores, offering promising leads for next-generation antidiabetic therapeutics. Isatin derivatives show strong potential as multifunctional antidiabetic agents by effectively inhibiting α- glucosidase and α-amylase. Their structural versatility enables enhanced enzyme inhibition, supporting their promise as lead compounds for postprandial glucose control.

Type 2 diabetes mellitus (T2DM) is a multifactorial metabolic disorder characterized by insulin resistance, progressive β-cell dysfunction, and chronic hyperglycemia, leading to microvascular and macrovascular complications. Among current therapeutic strategies, inhibition of intestinal carbohydrate-hydrolyzing enzymes, namely α-amylase and α-glucosidase, represents an established approach for controlling postprandial glucose excursions. However, clinically used inhibitors such as acarbose, miglitol, and voglibose are associated with gastrointestinal intolerance, limited selectivity, and variable efficacy, highlighting the need for improved therapeutic agents. In this context, 1,3,4-oxadiazole-based scaffolds have emerged as promising pharmacophores in antidiabetic drug design. The 1,3,4-oxadiazole ring acts as a bioisostere of amide and ester functionalities, conferring favorable physicochemical properties including enhanced metabolic stability, lipophilicity, and hydrogen-bonding capability. Structure-activity relationship (SAR) studies reported between 2015 and 2025 demonstrate that substituent effects-particularly phenolic hydroxyl groups, halogenation patterns, and extended aromatic systems-play a critical role in optimizing enzyme inhibition through hydrogen bonding, hydrophobic interactions, and π-π stacking within catalytic sites. While several oxadiazole-based hybrids incorporating privileged scaffolds such as benzimidazole, coumarin, and thiazolidinedione have been explored, evidence supporting modulation of additional targets including protein tyrosine phosphatase 1B (PTP1B), aldose reductase (ALR2), and inflammatory pathways such as the NLRP3 inflammasome remains limited and primarily derived from in vitro or computational studies. Computational approaches, including molecular docking, 3D-QSAR, and molecular dynamics simulations, have supported the rational optimization of oxadiazole derivatives, although their predictive limitations should be acknowledged. Despite encouraging preclinical findings, clinical translation remains constrained by insufficient pharmacokinetic, toxicological, and long-term efficacy data. Future research should prioritize systematic ADMET evaluation, standardized biological validation, and mechanistic expansion beyond carbohydrase inhibition to enable the development of clinically viable oxadiazole-based antidiabetic agents.

For prehospital providers, there are several causes for the patient with altered mental status or being unconscious to include the mnemonic AEIOU TIPS for Alcohol and acidosis, Endocrine, Epilepsy, Electrolytes, Encephalopathy, Infection, Opiates, Overdose, Uremia, Underdose, Trauma (head injury and blood loss), Insulin, Poisoning, Psychosis, Stroke, Seizure, and Syncope. Hypoglycemia, or low blood glucose level, is one of the most common causes of altered mental status for patients with and without diabetes. Estimates are that 1 to 2 percent of prehospital encounters and 7% of refusals are for hypoglycemia.[1][2] Diabetes mellitus often referred to as "diabetes" or "sugar" by laypeople is the most common endocrine disorder where the body does not either produce enough insulin or has a resistance to the circulating insulin, and is characterized by high blood sugar levels over prolonged periods. The three main types of diabetes are type I (previously referred to as insulin-dependent diabetes mellitus (IDDM) or juvenile-onset), type II (formerly referred to as non-insulin-dependent diabetes mellitus (NIDDM) or adult-onset) and gestational.[3] Insulin is an anabolic hormone produced in the beta cells of the pancreatic islets. The primary function of insulin is to regulate the metabolism of carbohydrates, protein, and fat through the absorption of glucose sugar from the blood into the liver, fat, and skeletal muscle cells. These small glucose molecules are then converted into larger molecules and stored for later usage.  Type 1 Diabetes Mellitus (T1DM) [4] This condition is commonly referred to as insulin-dependent diabetes or previously as childhood/adolescent-onset diabetes. It accounts for approximately 5% to 10% of all cases of diabetes. It can be related to the autoimmune destruction of insulin-producing beta cells in the islets of the pancreas from different causes, including genetic susceptivity, viral illness, toxins or alcohol-induced pancreatitis, or some dietary factors.  The age group affected is usually children and adolescents, but adults can develop T1DM.  As the name "insulin-dependent" implies, it requires the administration of subcutaneous insulin via intermittent injections or pump infusion. Type 2 Diabetes Mellitus (T2DM) [4] [5] This variant is commonly referred to as non-insulin diabetes or previously as adult-onset diabetes.  It accounts for approximately 90% of all cases of diabetes.  It is related to the desensitization of insulin/insulin resistance (response to insulin becomes diminished) in various tissues. Initially, there is an increase in insulin production, but this decreases over time. T2DM most commonly presents in persons older than 45 years, but it increasingly occurs in children, adolescents, and younger adults due to rising levels of obesity, physical inactivity, and energy-dense diets. Gestational Diabetes Mellitus (GDM) Hyperglycemia first detected during pregnancy is classified as gestational diabetes mellitus (GDM). Although it can occur anytime during pregnancy, GDM generally affects pregnant women during the second and third trimesters. According to the American Diabetes Association (ADA), GDM complicates 7% of all pregnancies. Women with GDM and their offspring have an increased risk of developing type 2 diabetes mellitus in the future.  Types of Therapies [6] Therapy for hyperglycemia is dependant upon how much insulin the body is producing, or how well that insulin that is produced is working.  The majority of patients with T2DM will be on an oral type of medication.  Patients with T1DM will be on a combination of either long-acting insulin with short-acting as a sliding scale, intermediate-acting insulin with short-acting insulin as a sliding scale, combination premixed insulin, or be on an insulin pump. Hypoglycemia  One of the public health concerns for diabetes mellitus and unintended complications for the reduction in blood glucose levels is hypoglycemia.[1][7][8] Hypoglycemia, by definition, is a plasma glucose concentration below 70 mg/dL, with most patients not having signs or symptoms until the plasma glucose concentrations drop below 55 mg/dL.[9]  Low plasma glucose concentration that requires assistance from another individual qualifies as severe hypoglycemia, and by context, all EMS encounters fall into this category.[1][10]  There has been an increase in morbidity (poor quality of life, series falls or car accidents, dementia, and hospitalization) and mortality related to severe hypoglycemia.[1]  Based upon 2015 data from NEMSIS, prehospital activation for diabetes-related cases accounts for about 2.3% of all activations, with the primary EMS encounter being hypoglycemia.[11] Patients experiencing severe hypoglycemia or other diabetes-related concerns typically reach out for help by calling 911 who may dispatch a fire department or EMS agency depending on the jurisdiction. Care is then provided by the first on scene provider who may be an emergency medical technician (EMT) intermediate or advanced emergency medical technician (IEMT or AEMT) or paramedic. Treat and release versus transport of patients with hypoglycemia by EMS providers depends on the clinical guidelines or protocols from either the state or regional level by medical directors. It is important to understand that patients with severe hypoglycemia are at a higher risk of having complications related to the hypoglycemic state.  Villani et al. indicated that approximately 50% of patients with severe hypoglycemia required transport to the hospital, and of that group, 41.3% were admitted to the hospital.[12][13][14]  One of the first studies on the topic of treating and releasing hypoglycemic patients in the field was "Development and Evaluation of Criteria Allowing Paramedics to Treat and Release Patients Presenting with Hypoglycemia: A Retrospective Study" published in Prehospital and Disaster Medicine in 1991.   Their study design proposed five criteria that would allow for appropriate release that would not require additional treatment from prehospital providers to include: 1. The patient has a history of type 1 or type 2 diabetes. 2. Blood sugar prior to treatment is below 4.4 mmol/L or 80 mg/dL. 3. Blood sugar after treatment is equal to or greater than 4.4 mmol/L or 80 mg/dL. 4. The patient has a normal mental status within 10 minutes of treatment.  5. The patient does not have any other complicating factors that required ED evaluation to such alcohol, chest pain, dyspnea, injuries related to falls, and/or renal dialysis. A 2016 study by Rostykus et al. reviewed prehospital clinical guidelines for hypoglycemia in 185 EMS agencies in the United States.  It revealed that less than half allowed for non-transport of patients with hypoglycemia after the low plasma glucose level was corrected.[15] A study by Moffet et al. looked at hypoglycemic patients treated by EMS in Alameda County from 2013 to 2015 and found that the transport rate was 13.5%. The demographic trends for those non-transported patients were adult patients < 60 years of age, males, finger stick blood glucose levels > 60 mg/dl, and EMS arrival times between 1800 and 0600.[1]

Limited data indicate that miglitol is poorly excreted into breastmilk. Because miglitol is also poorly absorbed orally, it is unlikely to adversely affect the breastfed infant.

TAS2R38 is a taste receptor gene located on human chromosome 7 that influences sensitivity to bitter tastes and has been implicated in innate immunity, glucose level, and human longevity. However, its potential association with Alzheimer's Disease (AD) has not been explored. Identifying such a genetic connection could support developing new drugs or repurposing existing ones for AD treatment.

Inhibition of carbohydrate digesting enzymes like pancreatic α-amylase has proven to be an effective strategy in countering postprandial hyperglycaemia. However, unwanted adverse effects associated with current therapies, such as ACB, voglibose, and miglitol, have necessitated the development of safer and more effective alternatives. These challenges have prompted growing interest in natural product scaffolds, particularly phenolic compounds, which exhibit favourable safety profiles and multifaceted enzyme inhibition. In this context, the strategic design of molecular conjugates integrating bioactive phenolics offers a promising route to enhance inhibitory potency, binding specificity, and therapeutic relevance. Reportedly, molecular hybrids with chalcone, phenolic acid, coumarin and polyphenol backbone exhibited impressive antioxidant and potent inhibition against digestive enzymes. In the quest of developing novel anti-hyperglycaemic agents, we report in vitro and in silico evaluation of the novel TPAC as potential pancreatic α-amylase inhibitors in the present work. Among the ten conjugates, T5 (IC50 = 50.65 ± 0.76 µM) exhibited strong inhibition against PPA which was comparable to positive control ACB (24.81 ± 0.98 µM). Computational binding analysis revealed binding of T5 to HPA (PDB ID: 2QV4) by interacting with the amino acid residues and distorting the receptor's catalytic site conformation. Furthermore, the conformational dynamic studies and electron density driven simulations established the stability and high reactivity of T5 within the ligand-receptor complex. The in silico studies corroborated the in vitro enzyme inhibition results, reinforcing the mechanistic insights into ligand-receptor interactions. Taken together, the experimental and computational results indicate that T5 merits further investigation as a candidate molecule targeting pancreatic α-amylase for the management of T2D .

We report a case in which low-dose prednisolone (LD-PSL) and an absorbable α-glucosidase inhibitor (miglitol) effectively treated reactive hypoglycemia caused by late dumping syndrome. A 73-year-old man underwent gastric tube reconstruction after an esophagectomy for esophageal cancer. Frequent diet management and a non-absorbable α-glucosidase inhibitor (voglibose) were prescribed to treat the reactive hypoglycemia caused by late dumping syndrome; however, hypoglycemia persisted. Changing the treatment to LD-PSL and miglitol ameliorated the reactive hypoglycemia and led to a resolution of hypoglycemic symptoms. Treatment with LD-PSL and miglitol is an effective treatment option for late dumping syndrome which does not respond to either guideline-based dietary modifications or voglibose treatment.

Myxococcus xanthus is a ubiquitous, obligately aerobic soil bacterium. M. xanthus has many two-component systems that serve to regulate responses to environmental stimuli. One of these systems, the multicomponent signaling system named the NmpRSTU pathway has been demonstrated to regulate genes that are predicted to be important for oxygen utilization, including the gene mxan_5531, which encodes a predicted hemerythrin-like protein. The family of proteins that includes hemerythrin-like proteins binds oxygen coordinated by a di-iron cofactor. Based on the sequence alignments and predictive structural analysis, we have determined that M. xanthus encodes at least five other predicted hemerythrin-like proteins. Of these six proteins predicted to be hemerythrin-like proteins, four were biochemically confirmed to bind oxygen in vitro using UV-spectroscopy. Currently, little is known about M. xanthus oxygen-dependent phenotypes, and the role of these hemerythrin-like proteins in M. xanthus is undescribed. To assess possible in vivo function, we chose to examine the impact of heterologous expression in the industrially relevant bacterium Gluconobacter oxydans. This bacterial species is used industrially to oxidize sugars to produce vinegar, vitamin C, the anti-diabetic drug miglitol, and several other products. However, due to its high oxygen demand, G. oxydans has a relatively slow growth rate under industrial-scale conditions. The expression of five of the hemerythrin-like proteins in G. oxydans led to faster doubling times and increased cell densities.

Psychiatric disorders present a significant global health burden with limited effective medications. Observing the widespread comorbidities between diabetes and psychiatric disorders, we explored the potential of repurposing antidiabetic drug targets for psychiatric treatments. We identified 32 target genes of 60 antidiabetics and performed Mendelian randomization analyses using expression and protein quantitative trait loci data from brain tissues alongside summary data for seven psychiatric disorders. Additionally, we conducted colocalization analyses, replication analyses in blood and at the single-cell level, single-cell gene annotation, developmental trajectory analysis, and various functional assessments. We found that elevated GANC expression in the putamen basal ganglia, nucleus accumbens basal ganglia, cortex, and whole blood was associated with a reduced risk of bipolar disorder (OR, 0.532-0.877; P, 4.04 × 10[-5] to 1.45 × 10[-7]), implying that antagonism of GANC by the antidiabetic drug miglitol could increase bipolar risk. Conversely, increased ABCC8 expression in the cortex, cerebellum, cerebellar hemisphere, and VIP- and LAMP5-expressing inhibitory neurons was linked to a higher risk of schizophrenia (OR, 1.054-1.119; P, 1.46 × 10[-3] to 4.42 × 10[-5]), suggesting that ABCC8 inhibition by sulfonylureas or glinides may lower the risk of schizophrenia. Colocalization analysis further confirmed the above associations. GANC and ABCC8 displayed specific developmental trajectories, and functional analyses revealed that they affected psychiatric risk through pathways related to potassium ion channels, insulin secretion, and glucose metabolism. Our findings highlight GANC and ABCC8 as potential targets, suggesting caution in miglitol use for bipolar disorder and the potential repurposing of sulfonylureas and glinides for schizophrenia.
