- CONCLUSIONS: First complete IVF cycle outcomes stratify prognosis for subsequent success, particularly in women < 38 years, while age overtakes as predominant predictor in older women. Biochemical pregnancy was not confirmed as an independent predictor in the main multivariable model. These findings support cautious, individualised counselling after the first complete IVF cycle in contemporary practice, while underscoring that prognostic estimates are conditional on completing a second cycle and should be validated in larger, independent cohorts.

- CONCLUSIONS: Relative Sertoli-cell FSH hyporesponsiveness represents a biologically plausible but currently unproven contributor to elevated SDF. The proposed FERTILE framework is intended as a conceptual research tool for biological stratification and hypothesis generation rather than a clinical decision-making instrument. Prospective studies incorporating endocrine profiling, pharmacogenetic characterization, standardized SDF assessment, and direct or validated surrogate measures of Sertoli-cell responsiveness are required to determine whether relative Sertoli-cell FSH hyporesponsiveness has independent clinical relevance for sperm DNA integrity and reproductive outcomes.

- CONCLUSIONS: In this cohort, patients with IBD undergoing IVF with PGT-A had euploid blastocyst rates similar to those without IBD, suggesting no clear impact on embryo chromosomal competence. However, given the retrospective design and small sample size, these findings should be interpreted with caution, and a modest effect cannot be excluded. Overall, the results provide some clinical reassurance and support ART as a reasonable option for patients with IBD, while underscoring the need for prospective studies to further evaluate disease-related factors.

- CONCLUSIONS: Our findings expand the OI mutational spectrum and suggest that reproductive decision-making may vary according to the sex of the affected parent, inheritance patterns, and phenotypic severity. A high cesarean section rate and skeletal complications were observed among women with OI during pregnancy, indicating that maternal and obstetric outcomes warrant careful attention in this population. Moreover, PGT was associated with healthy live births in this cohort and may reduce the likelihood of affected pregnancies in selected families.
