- Comparative efficacy of first-line antiseizure monotherapy in pediatric focal epilepsy: An umbrella review of systematic reviews and meta-analyses. [Review]Epileptic Disord. 2026 Oct 28. [Online ahead of print]ED
- CONCLUSIONS: Current umbrella-level evidence does not establish clear superiority of one first-line ASM over another for seizure control in pediatric focal epilepsy. Importantly, the absence of statistically significant differences should not be interpreted as evidence of therapeutic equivalence, because several estimates remain imprecise and clinically meaningful differences cannot be excluded. Interpretation is further limited by the scarcity of pediatric-specific comparative data, the frequent inclusion of mixed-age populations, and methodological and outcome heterogeneity across reviews. Safety and tolerability should therefore be considered alongside efficacy when individualizing treatment selection. Well-designed pediatric-specific comparative studies using standardized outcome measures are needed to strengthen evidence-based treatment recommendations.

- Clinical efficacy and cost-effectiveness of epilepsy surgery in low- and middle-income countries: A systematic review and meta-analysis. [Review]Epilepsia Open. 2026 Oct 28. [Online ahead of print]EO
- CONCLUSIONS: Resective epilepsy surgery was associated with substantially better seizure outcomes in appropriately selected patients in LMICs. However, the observational clinical evidence and heterogeneous, limited economic data preclude causal interpretation and broad conclusions on cost-effectiveness across settings.Epilepsy surgery can help people whose seizures remain uncontrolled despite medication, but access to surgery is limited in low- and middle-income countries. This review examined 10 studies involving 2587 patients from seven countries. Patients who underwent surgery were more likely to experience favorable seizure outcomes than those receiving medication alone. Surgical costs varied widely between countries, and evidence about whether surgery provides good value for money remains limited and inconsistent. These findings suggest that expanding access to epilepsy surgery may benefit appropriately selected patients, while further research is needed to understand its affordability and long-term economic value.

- Neonatal-onset developmental and epileptic encephalopathy: High genetic yield and long-term outcomes in a multicenter southern European cohort. [Journal Article]Epilepsia Open. 2026 Oct 28. [Online ahead of print]EO
- CONCLUSIONS: This large European cohort demonstrates a high genetic diagnostic yield in neonatal-onset DEE, supporting first-line whole-exome sequencing as the preferred first-tier genetic test. The persistence of severe neurodevelopmental impairment despite seizure control highlights the need for precision therapies targeting disease mechanisms in addition to seizure management. We studied 62 newborns who developed severe epilepsy caused by genetic changes, following them for up to 25 years. We found a genetic cause in over 80% of cases, and genetic testing that reads the entire genetic code was far more effective than tests that only look at specific genes. Although seizures improved in some children over time, nearly all had lasting intellectual disability regardless of seizure control. This suggests that the underlying genetic condition may contribute substantially to brain development in addition to the effects of seizures, although our study cannot determine the relative contribution of each factor.

- Unilateral Approach for Bilateral Posterior Cingulate Coverage in Stereoelectroencephalography: A Technical Note and Case Series. [Journal Article]Oper Neurosurg. 2026 Oct 28. [Online ahead of print]ON
- CONCLUSIONS: We have shown that the unilateral transhemispheric approach for bilateral PCC SEEG is feasible to sample both sides. Importantly, the transfalcine approach used, and previously considered a relative contraindication, was performed safely and accurately in our series. We demonstrated the utility of sampling both ipsilateral and contralateral PCC to identify the epilepsy network. Thus, unilateral SEEG for bilateral coverage, including transfalcine, provides an approach to consider, when exploring the PCC and other midline cortical regions.

- Amygdala volume and psychiatric burden in focal epilepsy: PET evidence for a glial substrate. [Journal Article]Brain Commun. 2026; 8(5):fcag394.BC
- Temporal lobe epilepsy is of heterogeneous pathology. While many patients present with hippocampal sclerosis and ipsilateral mesiotemporal atrophy, a subgroup of patients without human-detectable sclerosis via structural MRI are characterized by ipsilateral amygdala enlargement and worse postsurgical outcomes. A bidirectional relationship between temporal lobe epilepsy and depression is well-esta…

- Most antiseizure medicines act on a limited set of neuronal targets. Approximately a third of patients have treatment-resistant epilepsy, creating a need for mechanistically distinct drug targets. Human genetics can prioritise them, but epilepsy target-discovery has measured expression in plasma or bulk brain tissue and has yielded inconsistent candidate lists. Common-variant association for gene…

- Ischaemic stroke (IS) represents a leading cause of adult mortality and long-term disability globally, and post-stroke epilepsy (PSE) constitutes a severe complication that worsens neurological recovery and reduces quality of life, yet reliable early predictive biomarkers remain insufficient.
