- Predicting 30-day mortality in upper gastrointestinal bleeding using APUA components and lactate: Development and internal validation. [Journal Article]Biomol Biomed. 2026 Oct 27. [Online ahead of print]BB
- Early mortality risk assessment is crucial for acute upper gastrointestinal bleeding (UGIB). This study sought to develop and internally validate a prediction model using age, pulse, urea, and albumin (APUA) components, and to assess the added value of admission lactate levels. This retrospective, single-center study included 700 adults presenting to the emergency department with suspected acute …

- Pearls, Pitfalls, and Complications of Surgical Management for Peptic Ulcer Disease. [Review]Surg Clin North Am. 2026 Dec; 106(6):841-853.SC
- Peptic ulcer disease management has evolved from predominantly elective surgical intervention to effective medical therapy with acid suppression and Helicobacter pylori eradication. Surgery is now reserved for the emergency treatment of severe refractory disease and acute complications. Upper endoscopy serves as the diagnostic gold standard and allows for therapeutic intervention. Operative manag…

- Venous Thromboembolism in Acute Necrotizing Pancreatitis Is Associated With Worse Outcomes, With Subtype-Specific Patterns: A Nationwide Analysis, 2016-2022. [Journal Article]Pancreas. 2026 Oct 22. [Online ahead of print]P
- CONCLUSIONS: VTE in ANP was associated with higher in-hospital mortality, systemic complications, and resource utilization, with substantial heterogeneity across subtypes. DVT-associated ANP showed higher rates of systemic complications, whereas PVT and SVT showed distinct territory-specific vascular-complication patterns. Prospective studies should evaluate phenotype-specific anticoagulation strategies.

- A gastric dieulafoy lesion presenting with life-threatening upper gastrointestinal hemorrhage during recovery from hypertriglyceridemic acute pancreatitis: a case report. [Case Reports]
- CONCLUSIONS: A gastric Dieulafoy lesion is a rare but life-threatening cause of bleeding during recovery from HTG-AP. A nondiagnostic first endoscopy because of clot retention does not exclude it, and persistent bleeding warrants repeat endoscopy after stabilization and gastric clearance. Bleeding should not be attributed solely to stress injury or anticoagulation; an underlying structural lesion should be sought. Clipping achieved durable hemostasis in this patient, but a single case cannot establish comparative efficacy.

- Umbilical cord ulceration complicating fetal upper gastrointestinal obstruction: a case report. [Case Reports]
- Umbilical cord ulceration (UCU) is an uncommon but devastating cause of acute fetal hemorrhage, often associated with fetal upper gastrointestinal obstruction. We report a woman aged 30-35 years (gravida 4, para 1) referred at 23 weeks of gestation for suspected fetal gastrointestinal abnormality. Prenatal ultrasonography showed an enlarged fetal stomach with a double-bubble sign, and subsequent …

- Ultrasound-guided glossopharyngeal nerve block for safe and effective anesthesia in a critically ill patient with acute myocardial infarction undergoing urgent endoscopy. [Case Reports]Agri. 2026 Oct 01; 38(4):302-305.AGRI
- Glossopharyngeal nerve block (GPNB) can provide effective anesthesia for procedures involving the oropharynx and upper airway. Ultrasound-guided GPNB (UGPNB) may be particularly useful when conventional sedation or topical anesthesia is undesirable because of the risk of respiratory or cardiovascular complications. In this article, we report the use of UGPNB as an alternative anesthetic technique…

- Octreotide Plus Proton pump inhibitor (PPI) versus PPI alone in Non-Variceal Upper gastrointestinal (GI) Bleeding in intensive care unit (ICU): A Multi-Center Randomized Trial. [Journal Article]Am J Med Sci. 2026 Oct 17. [Online ahead of print]AJ
- CONCLUSIONS: The addition of octreotide to PPI therapy did not improve bleeding-related outcomes. Prolonged prothrombin time did not correlate with increased bleeding or transfusion needs. These findings suggest no clinical benefit of adjunctive octreotide in this study and underscore the importance of appropriate patient selection.

- Successful endoscopic vacuum therapy in diffuse gastric hemorrhage secondary to acute ischemia: a case report. [Journal Article]IGIE. 2026 Sep; 5(3):388-392.IGIE
- Acute gastric ischemia is rare and may lead to diffuse mucosal bleeding, often refractory to conventional endoscopic therapy. We describe the case of a 42-year-old critically ill burn patient who developed persistent upper gastrointestinal hemorrhage unresponsive to medical treatment, in whom the novel use of endoscopic vacuum therapy (EVT) was undertaken. Serial endoscopies demonstrated progress…

- ISG Task Force on Upper Gastrointestinal Bleeding (UGIB)-Comparison of Risk Scores in Nonvariceal Bleeding-A Prospective Multicenter Study. [Journal Article]
- CONCLUSIONS: GBS was more accurate and had more desirable clinical characteristics in predicting the need for a transfusion for hospitalized patients than TRS. However, both GBS and TRS have poor to modest predictive capability for 30-day mortality and rebleeding. They do not, then, appear to be useful as screening tools to identify people at risk for those two outcomes in a hospital setting.

- Treatment Strategy for Gastric Variceal Bleeding Caused by Sinistral Portal Hypertension Based on Hemodynamic Characteristics: A Case Report. [Case Reports]
- Gastric varices secondary to sinistral portal hypertension (SPH) are a rare cause of upper gastrointestinal (GI) hemorrhage. Endoscopic histoacryl injection is widely used for endoscopic hemostasis in portal hypertension (PH) and is considered a rescue treatment for massive acute GI bleeding. However, it is associated with a high rate of recurrence. Herein, we report a case of repeated pancreatit…

- Long-term Trends in Upper Gastrointestinal Bleeding and Ischemic Outcomes after Myocardial Infarction. [Journal Article]Eur J Prev Cardiol. 2026 Sep 26. [Online ahead of print]EJ
- CONCLUSIONS: Bleeding hospitalizations after MI increased until 2013-14, with UGIB the most common source, while ischemic outcomes declined. These opposing trends were associated with the adoption of invasive management and intensified antithrombotic therapy. Routine tailoring of antithrombotic intensity and systematic gastroprotective measures should be considered in post-MI care.

- Necrotizing Oesophagitis Associated with Ketoacidosis: A Case Series Exploring the Diagnostic Dilemma of Metabolic Overlap Syndromes. [Journal Article]
- CONCLUSIONS: Acute esophageal necrosis should be considered when upper GI bleeding complicates ketoacidosis. The underlying etiology is often multifactorial, requiring clinicians to look beyond simple diagnostic labels. Recognizing distinct clinical patterns-overlap syndromes, pseudo-DKA, cytokine storms, and dual alcohol-related pathologies-may aid in early diagnosis and severity assessment in resource-limited settings.
