  • Neonate (IV/IM):
    • Postnatal age ≤7 days: 50 mg/kg/dose Q12 hr
    • Postnatal age >7–28 days:
      • <1000 g:
        • Postnatal age 8–14 days: 50 mg/kg/dose Q12 hr
        • Postnatal age 15–28 days: 50 mg/kg/dose Q8 hr
      • ≥1000 g: 50 mg/kg/dose Q8 hr
    • Meningitis:
      • Postnatal age ≤7 days: 50 mg/kg/dose Q8–12 hr
      • Postnatal age >7 days: 50 mg/kg/dose Q8 hr
  • Infant (>1 mo), child and adolescent (IV/IM):
    • Mild/moderate infection:100–150 mg/kg/24 hr ÷ Q8 hr; max. dose: 6 g/24 hr
    • Meningitis: 150–200 mg/kg/24 hr ÷ Q8 hr ( max. dose: 6 g/24 hr)
    • Serious Pseudomonas infections: 200–300 mg/kg/24 hr ÷ Q8 hr ( max. dose: 12 g/24 hr)

  • Use with caution in penicillin-allergic patients or in presence of renal impairment. Good Pseudomonas coverage and CSF penetration. May cause rash, liver enzyme elevations, and false-positive urine-reducing substance (e.g., Clinitest, Benedict's solution, or Fehling's solution) and Coombs test. Enzymatic glucose oxidase urinary glucose tests (e.g., Clinistix or Tes-Tape) are recommended. Probenecid increases serum ceftazidime levels. Adjust dose in renal failure (see Chapter 31 ). Nonconvulsive status epilepticus, neuromuscular excitability, and myoclonia may occur with elevated levels of ceftazidime.
