  • CONCLUSIONS: Antiresorptive therapy should not be regarded as an absolute contraindication to implant therapy in appropriately selected patients, particularly those receiving osteoporosis-dose regimens. However, chronic peri-implant inflammation, repeated surgical intervention, and high-dose oncologic antiresorptive exposure may impair osteoimmune adaptation and increase susceptibility to delayed healing and MRONJ-related complications.

Current evidence supports an association between oral dysbiosis, chronic oral inflammation, and pathways relevant to neurodegeneration, particularly for periodontitis. However, important knowledge gaps remain, especially concerning peri-implant diseases, where human data are limited.

However, a sustainable clinical translation requires responsible implementation of the AI and a stronger data set that represents the multimodal nature of the periodontal and peri-implant diseases. Moving forward, the data sets being used to train the models need to be multimodal, multicenter, and clinically verified to support the generalizability of the findings. In addition, there needs to be clear ethical safeguards and clinician oversight to make sure the results are interpreted in the correct way.
