- A prospective natural history study protocol for clinical trial readiness in synaptic disorders. [Journal Article]Epilepsia. 2026 Jul 14. [Online ahead of print]E
- CONCLUSIONS: In summary, we present a feasible natural history protocol with prospective data for two complex neurodevelopmental disorders with natural histories that have previously been incompletely characterized, within a regulatory framework that will support the use of these data to expedite clinical trial development.

- Stereo-electroencephalography-guided responsive neuromodulation: A propensity-matched cohort study. [Journal Article]Epilepsia. 2026 Jul 14. [Online ahead of print]E
- CONCLUSIONS: In a climate of growing delays in epilepsy care, we did not observe differences in RNS efficacy with or without prior sEEG monitoring in patients with unilateral or bilateral temporal lobe epilepsy, temporal-plus epilepsy, and extratemporal epilepsy. Although prospective data are needed to investigate the relative benefit of preoperative sEEG use, these findings may help guide patient selection and optimize definitive surgical planning.

- New onset refractory status epilepticus (NORSE) versus refractory status epilepticus not meeting NORSE criteria: A comparative clinical and electroencephalography-based study. [Journal Article]Epilepsia. 2026 Jul 14. [Online ahead of print]E
- CONCLUSIONS: NORSE, particularly c-NORSE, tends to follow a more severe course than RSE not meeting NORSE criteria, with more severe background EEG features including the initial EEG, greater seizure burden, longer ITU stays, and increased incidence of cognitive impairment at discharge. Six-month mortality was similar, implying additional contributing factors to mortality.

- Obstructive sleep apnea affects interictal epileptiform discharges in patients with focal epilepsy. [Journal Article]Epilepsia. 2026 Jul 14. [Online ahead of print]E
- CONCLUSIONS: RE are associated with dynamic perievent modulation of IED activity in patients with focal epilepsy, especially those with TLE, providing novel insight into the relationship between OSA and epilepsy.

- Socioeconomic status and epilepsy surgery in the United States: Timing and outcomes. [Journal Article]Epilepsia. 2026 Jul 13. [Online ahead of print]E
- CONCLUSIONS: These results underscore the need for equitable access to epilepsy surgery and suggest that socioeconomic status should not delay or deter surgical referral, as outcomes remain robust across diverse populations. We also demonstrate that patients enter surgery on distinct health trajectories shaped by socioeconomic disadvantage, highlighting the need for early identification and intervention to maximize postoperative benefit.

- Neuropsychological functioning and progression in Lafora disease. [Journal Article]Epilepsia. 2026 Jul 10. [Online ahead of print]E
- CONCLUSIONS: LD is characterized by prominent executive dysfunction accompanied by reduced adaptive communication skills, even in the initial stages. The CCTT was sensitive in detecting early deficits and cognitive decline, supporting its use in clinical practice and research. These findings highlight the need for standardized neuropsychological testing and support its role in tracking disease progression and as an outcome measure in clinical trials.

- Do witnesses of functional/dissociative seizures recall them accurately? A UK experimental study of semiology recall at clinically relevant time points with a pilot intervention. [Journal Article]Epilepsia. 2026 Jul 10. [Online ahead of print]E
- CONCLUSIONS: Eyewitness recall of FDSs is modest and declines over clinically relevant intervals, and confidence poorly reflects accuracy. Early structured questioning did not improve retention. These findings highlight a key constraint on diagnostic accuracy in TLOC. Strategies to support differentiation in routine care are needed.

- Obstructive sleep apnea in people with epilepsy: Modifying risk. [Review]Epilepsia. 2026 Jul 09. [Online ahead of print]E
- Obstructive sleep apnea (OSA) is a common but underdiagnosed and undertreated sleep disorder among people with epilepsy (PWE). In PWE, this sleep disorder is often managed as a comorbid condition rather than a contributor to epilepsy outcomes. For many years, OSA has been associated with higher seizure burden and interictal epileptiform discharges. Emerging evidence links OSA to late onset epilep…

- Complementary predictive value of electromagnetic source imaging and hemodynamic responses in epilepsy surgery: A quantitative spatial analysis. [Journal Article]Epilepsia. 2026 Jul 09. [Online ahead of print]E
- CONCLUSIONS: EEG/MEG and EEG/fMRI showed complementary profiles. Resection of EEG/MEG localizations were more frequently associated with good outcome, whereas unresected EEG/fMRI responses predicted poor outcome. Concordant and resected multimodal findings were associated with good outcome, supporting a quantitative multimodal approach in presurgical evaluation.

- Modified Atkins diet therapy in adult drug-resistant epilepsy: Quality of life and mental health outcomes. [Journal Article]Epilepsia. 2026 Jul 09. [Online ahead of print]E
- CONCLUSIONS: In this single-arm prospective cohort study, modified Atkins diet therapy was associated with improved quality of life and reduced anxiety symptoms in adults with DRE, independent of major changes in seizure frequency.

- Information needs of patients considering epilepsy surgery: A scoping review. [Review]Epilepsia. 2026 Jul 08. [Online ahead of print]E
- CONCLUSIONS: This review highlights critical gaps in the quality and accessibility of information for individuals considering epilepsy surgery. Patient testimonials emerged as a powerful tool in supporting informed decision-making. Future efforts should focus on developing culturally responsive, readable, and patient-centered resources tailored to diverse epilepsy populations.

- Psychological stress during wartime and its association with seizure-related emergency department utilization. [Journal Article]Epilepsia. 2026 Jul 08. [Online ahead of print]E
- CONCLUSIONS: War-related stress correlated with stable seizure-related acute care utilization, with minor redistribution among recurrent ED users. Older age and dementia identify vulnerable subgroups requiring enhanced monitoring during national emergencies. Targeted monitoring and clinical support for high-risk patients with epilepsy may help reduce acute seizure-related ED utilization during high-stress situations.

- WONOEP appraisal: Biomarkers and treatment strategies beyond the synapse. [Review]Epilepsia. 2026 Jul 08. [Online ahead of print]E
- Epilepsy is a heterogeneous neurological disorder affecting more than 70 million people worldwide, posing significant challenges for clinicians due to its complex etiology, diverse manifestations, variable treatment responses, and the inability to predict seizures or disease onset reliably. Despite advances in antiseizure medications, approximately 30% of patients remain treatment-resistant, high…

- CONCLUSIONS: ECoG-guided resection is associated with better long-term SF in patients with CCM-related epilepsy. Our evidence shows that ECoG may be a useful adjunct in cases of CCM-related epilepsy surgery, particularly in extratemporal lesions.
