- Utilization of Hospice and Palliative Care Among Patients With Mental Illness: A Retrospective Cohort Study. [Journal Article]J Am Geriatr Soc. 2026 Jul 11. [Online ahead of print]JA
- CONCLUSIONS: Mental illnesses were associated with complex hospice utilization patterns characterized by longer length-of-stay, higher likelihood of enrollment far from death, lower likelihood of enrollment within 180 days before death, and lower palliative care consultation use in some groups.

- Prolonged Use of Postoperative Gabapentin After Discharge From a Skilled Nursing Facility. [Journal Article]J Am Geriatr Soc. 2026 Jul 11. [Online ahead of print]JA
- CONCLUSIONS: Of our cohort, 38% had prolonged use, more than 1.5 times the proportion of prolonged use found in a past study among patients being discharged directly home. Importantly, patients who were sicker and more disadvantaged were more likely to have prolonged use. Our study highlights that additional work needs to focus on understanding and preventing inappropriate prolonged use of gabapentinoids after SNF discharge.

- Changes in Life Expectancy After Stroke Among Americans Aged 50 and Above by Sex, 1996-2018. [Journal Article]J Am Geriatr Soc. 2026 Jul 11. [Online ahead of print]JA
- CONCLUSIONS: Long-term consequences of stroke appear to be becoming less lethal, likely reflecting improvements in secondary prevention and post-stroke long-term care. Nonetheless, a gap of approximately 10 years in LE50 between stroke survivors and the general population persisted in 2008-2018. Continued investment in both acute stroke care and long-term secondary prevention remains essential to further close this gap.

- The Impact of Recorded Music on Postoperative Delirium in Older Adults After Hip Fracture Surgery (IPOD): A Quasi-Experimental Study. [Journal Article]J Am Geriatr Soc. 2026 Jul 11. [Online ahead of print]JA
- CONCLUSIONS: Music influenced the course of postoperative delirium by reducing its intensity and lowered postoperative pain within the intervention group. Future research should build on these findings by implementing a 30-min, once-daily music intervention during the first 3 postoperative days.

- Hearing Aid Use Attenuates the Association Between Hearing Loss and Frailty: A 2003-2018 NHANES Cross Sectional Analysis. [Journal Article]J Am Geriatr Soc. 2026 Jul 09. [Online ahead of print]JA
- CONCLUSIONS: This study demonstrated a positive association between hearing threshold and frailty, such that poorer hearing (higher thresholds) was associated with greater frailty. Hearing aid use attenuated this association, highlighting the need for better access to hearing care and informing the development of frailty screening tools.

- Psychological Impact of Caregiving When Navigating Language Barriers in Health Care: A Scoping Review. [Review]J Am Geriatr Soc. 2026 Jul 08. [Online ahead of print]JA
- CONCLUSIONS: Caregivers supporting individuals who face language barriers experience significant psychological strain. Routine screening for caregiver burden, integration of professional interpreters, and expanded access to multilingual and culturally appropriate supports are recommended. Future research should employ quantitative and mixed-method designs to quantify psychological impact, investigate the intersection of language, gender, and other aspects of identity, and evaluate interventions that reduce caregiver burden. Addressing both linguistic and cultural dimensions is essential for improving caregiver well-being and promoting equitable healthcare delivery.

- Assessing Associations of Three Types of Higher-Risk Medication Use and Mortality in Older Adults. [Journal Article]J Am Geriatr Soc. 2026 Jul 08. [Online ahead of print]JA
- CONCLUSIONS: When considering multiple types of higher-risk medication exposure, only polypharmacy was consistently and independently associated with increased mortality among older adults. These results highlight the harms of polypharmacy specifically and could help further target deprescribing interventions to the most high-risk patients.

- An Electronic Health Record "Nudge" to Reduce Antipsychotic Medication Prescribing for Primary Care Patients With Dementia: A Pragmatic RCT. [Journal Article]J Am Geriatr Soc. 2026 Jul 07. [Online ahead of print]JA
- CONCLUSIONS: This EHR alert did not reduce the primary outcome of antipsychotic pill-days. However, the nonsignificant effect size was substantial, and exploratory analyses revealed that the intervention significantly reduced pill-days for providers with average baseline prescribing. Future alerts should test targeting high prescribers.

- Assigning Probable Dementia Status Using Routinely Collected Electronic Health Record Data. [Journal Article]J Am Geriatr Soc. 2026 Jul 06. [Online ahead of print]JA
- CONCLUSIONS: A probabilistic computational phenotype derived from routinely collected EHR data accurately reproduced clinician-adjudicated ADRD status and demonstrated net clinical benefit, including among ED patients whose ADRD was not captured by diagnosis codes. Adoption of this replicable framework may enable healthcare organizations to strengthen ADRD surveillance and reduce underdiagnosis.

- Caregiver Burden at the Onset of Acute Hospital-At-Home. [Journal Article]J Am Geriatr Soc. 2026 Jul 03. [Online ahead of print]JA
- CONCLUSIONS: High caregiver burden is common at the onset of acute HaH care. These exploratory findings suggest that caregiver vulnerability at admission is related to pre-existing caregiving context and psychosocial resources, supporting systematic caregiver assessment early in the HaH episode to identify individuals who may benefit from targeted support.
