Gluteal polymers may lead to chronic inflammation, recurrent infection, and progressive deformity, creating major challenges for diagnosis, surgical planning, and patient counseling. Although high-frequency dermatologic ultrasound is the gold standard for identifying and characterizing superficial aesthetic fillers, its diagnostic performance in the gluteal region is limited by insufficient penetration and reduced ability to differentiate deep subcutaneous and intramuscular compartments. Gluteal magnetic resonance imaging (MRI) overcomes these limitations by providing comprehensive multiplanar coverage, superior soft‑tissue contrast, and deep‑plane characterization. We present the case of a 42-year-old woman with recurrent inflammatory and infectious reactions following polymer injections performed in 2016. Despite multiple staged procedures and autologous fat grafting, chronic fibrosis and tethering persisted. Volumetric MRI allowed clear delineation of multilocular subcutaneous and superficial intramuscular polymer deposits, including inflammatory interfaces and postoperative scarring. Three‑dimensional (3D) segmentation was performed using 3D Slicer and enabled compartment‑specific segmentation with volumetric quantification of subcutaneous and intramuscular components. The resulting color‑coded 3D reconstruction provided high‑fidelity visualization of disease distribution and depth, aiding surgical strategy. Importantly, 3D segmentation enhanced patient counseling by illustrating, in a tangible and intuitive manner, the anatomical limitations that make complete polymer removal unfeasible-even with multiple surgical stages. This case highlighted the growing relevance of volumetric postcontrast MRI and 3D segmentation as practical and clinically impactful tools in the management of gluteal polymer complications. Their integration into routine imaging protocols may improve diagnostic accuracy, guide individualized surgical planning, align expectations, and support future developments in automated quantification and augmented‑reality surgical navigation.

Airbags are designed to reduce morbidity and mortality during motor vehicle collisions; however, rare manufacturing defects may result in catastrophic injuries. We report a unique case of a 45-year-old woman involved in a low-speed collision, who sustained projectile-induced airbag trauma. Deployment of a defective airbag expelled a metallic projectile, causing complex panfacial fractures, extensive maxillary bone loss, and a full-thickness palatal defect. The patient initially received suboptimal management, necessitating secondary reconstruction at a tertiary referral center. Revision surgery included panfacial fracture realignment, orbital floor reconstruction with bone grafting, and closure of the palatal defect using a temporalis musculofascial flap, achieving satisfactory functional and aesthetic outcomes. This case underscores the distinct and potentially lethal mechanism of airbag failure associated with recalled inflators; highlights the importance of early recognition, thorough reporting, and multidisciplinary management; and provides critical insights for improving patient safety and guiding clinicians encountering similar injuries.

Breast implant exposure is an uncommon but significant complication in aesthetic and reconstructive breast surgery. Salvage procedures aim to restore soft-tissue coverage while minimizing morbidity. The myocapsular flap offers vascularized tissue already present in the operative field, providing a potential alternative to more complex reconstructive options. A 37-year-old woman with a prior augmentation mastopexy underwent secondary mastopexy with implant exchange. Six weeks later, she developed wound dehiscence, localized infection, and partial left implant exposure. Intravenous antibiotics were initiated. Under general anesthesia, the implant was removed, the pocket irrigated and debrided, and an inferomedial rectangular myocapsular flap elevated and transposed to cover the defect. The sterilized implant was reinserted into the reconstructed pocket. The flap provided reliable vascularized coverage, allowing successful implant salvage. Healing was uneventful, with no recurrent infection or reexposure. The aesthetic result was satisfactory, with adequate contour and no donor-site morbidity. The myocapsular flap is a practical, low-morbidity option for implant salvage in cases of limited exposure, offering effective coverage within the same surgical field.

Soft-tissue sarcomas of the hand present significant reconstructive challenges. This is due to the complex anatomy and function of the hand, as well as limited local tissue availability. Reconstruction in pediatric patients adds further intricacy because of growth considerations. We present more than 10 years of follow-up of a neonatal congenital fibrosarcoma of the volar hand managed with a dermal regeneration template followed by skin grafting. After neoadjuvant chemotherapy and wide excision, a large full-thickness defect remained. Integra (Integra LifeSciences, Princeton, NJ) was applied for temporary coverage; however, wound vacuum-assisted closure was not feasible due to the patient's age. A split-thickness skin graft was initially placed but demonstrated graft loss. After debridement and thorough wound bed preparation, a full-thickness skin graft was successfully applied, providing stable, durable coverage. Long-term follow-up through skeletal maturity demonstrated excellent hand function, strength, sensation, and range of motion comparable to the contralateral side. The reconstruction had satisfactory cosmesis without hypertrophic scarring, contracture, or abnormal tissue growth at the level of the graft. The graft integrated smoothly with adjacent normal palmar tissue. Staged reconstruction using a dermal regeneration template followed by full-thickness skin grafting can achieve durable, functional, and aesthetic outcomes in pediatric oncological hand defects. Although intended as temporary coverage, Integra ultimately served as a satisfactory component of the definitive reconstruction.
