- Polysaccharides from Elsholtzia rugulosa: structural characteristics and treating functional dyspepsia via gut microbiome in conjunction with targeting the PI3K/Akt/cGMP-PKG pathways. [Journal Article]Front Nutr. 2026; 13:1833860.FN
- CONCLUSIONS: ERP may alleviate FD in association with gut microbiota modulation and changes in the PI3K/Akt/cGMP-PKG-related pathway. ERP-N also exhibits immunomodulatory activity, supporting the traditional use of Elsholtzia rugulosa in gastrointestinal health and offering potential for FD therapy development.

- Endoscopic Treatment of a Giant Gastric Bezoar: Sequential Use of Alligator Forceps and Snare Fragmentation. [Case Reports]
- We report the case of a 79-year-old man with a giant gastric diospyrobezoar (50 × 30 mm) who presented with epigastric pain. Two weeks of cola dissolution therapy at home was ineffective. During endoscopy, alligator forceps were used to create grooves on the hard, smooth surface of the bezoar, enabling a 30-mm polypectomy snare to fragment the bezoar into pieces smaller than 1 cm. Residual fragme…

- When Nausea and Vomiting Are Something Else Entirely, It's Not Gastroparesis: A Report of Two Cases. [Case Reports]
- Gastroparesis, a chronic motility disorder, is a diagnosis of exclusion that necessitates careful evaluation for structural and surgically correctable causes. Although the medical differential diagnosis emphasizes functional etiologies, anatomic abnormalities are often under-recognized despite guideline recommendations for thorough endoscopic and structural assessment. Symptoms such as nausea, vo…

- Predictors and consequences of perioperative blood transfusions in deep inferior epigastric perforator flap breast reconstruction. [Journal Article]J Plast Reconstr Aesthet Surg. 2026 Aug 06; 121:143-151. [Online ahead of print]JP
- CONCLUSIONS: Bleeding disorders, diabetes, bilateral reconstruction, lower preoperative hemoglobin, urgent reoperation, and hematoma were independently associated with increased transfusion risk. Perioperative transfusions were independently associated with donor site dehiscence. These findings highlight the importance of preoperative optimization and careful intraoperative fluid management, and underscore that blood transfusions, although often necessary, are not without consequences.

- Berotralstat Tolerability and Effectiveness in Hereditary Angioedema: Berolife Study Results. [Journal Article]
- CONCLUSIONS: Berotralstat was generally well tolerated with a tolerability profile consistent with prior clinical trials. Significant reductions in monthly HAE attack rates were observed, confirming its effectiveness in real-world settings.

- Infliximab-Associated IgA Vasculitis Masquerading as Crohn's Disease Relapse: Leukocytoclastic Vasculitis Confirmed on Duodenal Biopsy - A Case Report. [Case Reports]
- CONCLUSIONS: This case highlights IgA vasculitis as a critical diagnostic consideration in patients with IBD receiving anti-TNF-α therapy who present with acute abdominal symptoms, especially when GI involvement precedes the appearance of purpura ("herald enteritis"). Biopsy of the duodenal ulcer base may provide decisive clues for differentiating vasculitis from CD relapse, thereby preventing inappropriate intensification of CD treatment.

- When colon cancer crosses boundaries: acute abdomen from direct pancreatic infiltration by a splenic flexure tumour. [Case Reports]
- Colorectal cancer is the second-leading cause of cancer death in the United States. Locally advanced invasion into an adjacent organ (T4b) occurs in 10%-20% of colon cancers. A 65-year-old man presented with weakness and abdominal pain. The abdomen was soft, moderately distended, and severely tender in the epigastric region. Laboratory studies showed leukocytosis and lactic acidosis. Computed tom…

- Deep Inferior Epigastric Perforator Flap Reconstruction of Breast Necrobiosis Lipoidica. [Case Reports]
- Necrobiosis lipoidica (NL) is a granulomatous disease presenting as plaques with potential for progression to chronic ulcerating lesions. Breast involvement of the disease is rare. There are currently no FDA-approved medical therapies, and surgical treatment is controversial due to concerns for disease recurrence. Limited historical cases of surgical management demonstrate success when excision i…

- The genetic association between celiac disease and risk of functional dyspepsia: A Mendelian randomization study. [Journal Article]
- Celiac disease (CeD) is a common autoimmune disease of the small intestine triggered by ingesting gluten-containing foods. Chronic symptoms are characteristic of functional dyspepsia (FD), including chronic epigastric pain and burning. The clinical and genetic overlap between FD and CeD is currently being investigated in a large-scale comorbidity and genetic predisposition study. In this study, c…

- Gastric lipoma causing ball-valve syndrome successfully treated by endoscopic submucosal dissection. [Journal Article]
- Ball-valve syndrome (BVS) is a rare condition characterized by intermittent gastric outlet obstruction caused by a mobile gastric lesion. Large gastric lipomas associated with BVS have traditionally been managed surgically; however, minimally invasive endoscopic treatment may be feasible in selected cases. We report a case of a 52-year-old woman presenting with a 1-month history of postprandial e…

- Pancreatic tuberculosis: Likely originating from peripancreatic tuberculous lymphadenitis. [Review]Hepatobiliary Pancreat Dis Int. 2026 Aug 05. [Online ahead of print]HP
- CONCLUSIONS: Only a small proportion of patients with pancreatic tuberculosis are related to miliary tuberculosis. Pancreatic tuberculosis is more likely to originate from peripancreatic tuberculous lymphadenitis.

- Peri-operative CDK4/6 inhibitors in DIEP flap breast reconstruction: Early safety data and proposed management strategy. [Journal Article]J Plast Reconstr Aesthet Surg. 2026 Aug 06; 121:139-142. [Online ahead of print]JP
- The introduction of cyclin-dependent kinase 4/6 (CDK4/6) inhibitors has significantly altered the treatment paradigm for hormone receptor-positive, HER2-negative breast cancer. Increasingly, surgeons encounter patients receiving these agents at the time of breast reconstruction, yet there is no published evidence on the safety of free tissue transfer in this context. We present the first series e…

- A Pancreas Out of Place: Acute Interstitial Pancreatitis Caused by Pancreatic Herniation in a Type IV Paraesophageal Hernia. [Case Reports]
- Pancreatic involvement in paraesophageal hernias is rare and can precipitate acute pancreatitis through mechanical compression or ischemia. We report an 82-year-old woman who presented with epigastric pain and an elevated lipase level. Imaging demonstrated a large type IV paraesophageal hernia containing the stomach, transverse colon, and pancreatic body and tail, with inflammatory changes locali…

- A Fibroblast Activation Protein Inhibitor (FAPI)-Avid Gastric Antral Mass Mimicking Advanced Gastric Cancer: A Case of Presumed Inflamed Gastric Ectopic Pancreas. [Case Reports]
- Gastric ectopic pancreas is an uncommon congenital anomaly that is usually asymptomatic but can mimic gastric malignancy when inflamed. We report the case of a 33-year-old previously healthy woman who presented with acute severe epigastric pain and inability to tolerate oral intake. Endoscopy demonstrated annular thickening of the gastric antrum, congested and friable mucosa, and poor gastric dis…
