- Intravenous Tirofiban After Tenecteplase in Acute Ischemic Stroke: The INSTANT Randomized Clinical Trial. [Journal Article]JAMA. 2026 May 08. [Online ahead of print]JAMA
- CONCLUSIONS: Among patients with acute ischemic stroke without large or medium vessel occlusion or a cardioembolic source who had an inadequate clinical response to intravenous tenecteplase, adjunctive intravenous tirofiban increased the likelihood of an excellent outcome at 90 days.

- Large vessel occlusions associated with ipsilateral high-grade extracranial internal carotid artery stenosis, also called tandem occlusions, remain under investigation for optimal management. The risk of in-stent thrombosis is heightened for those who undergo carotid stenting in conjunction with mechanical thrombectomy (MT). Tirofiban, an intravenous antiplatelet agent functioning by binding the …

- Efficacy and safety of early intravenous administration of tirofiban after thrombolysis in acute ischemic stroke: a systematic review and meta-analysis of randomized controlled trials. [Review]
- Intravenous thrombolysis (IVT) is the cornerstone of reperfusion therapy for acute ischemic stroke (AIS), yet only a minority of patients achieve functional independence with vessel re-occlusion in up to one-third of cases. IVT targets fibrin but does not inhibit platelet aggregation, providing a rationale for adjunctive early platelet inhibition; so in this context tirofiban, a short-acting glyc…

- Prophylactic tirofiban versus oral dual antiplatelet therapy in patients undergoing endovascular treatment of intracranial aneurysms: A meta-analysis. [Meta-Analysis]Medicine (Baltimore). 2026 Apr 17; 105(16):e48070.M
- CONCLUSIONS: These findings support the prophylactic administration of tirofiban to patients undergoing endovascular treatment of intracranial aneurysms, as it was associated with a significantly lower incidence of thromboembolic events without increasing the risk of hemorrhagic events. More studies are needed to determine differences between prophylactic monotherapy with tirofiban and prophylactic tirofiban in combination with DAP therapy.

- Tirofiban combined with aspirin for branch atheromatous disease: a propensity score-matched study. [Journal Article]Front Neurol. 2026; 17:1802990.FN
- CONCLUSIONS: In this single-center cohort of Chinese patients with acute, mild-to-moderate BAD (NIHSS <15), tirofiban plus aspirin reduced END and improved 90-day functional outcomes. Exploratory evidence suggests it may also serve as a rescue therapy for patients deteriorating under DAPT. However, its efficacy and safety require validation in prospective, randomized studies.

- Emergency Intra-Arterial Thrombolysis for Parent Artery Thrombosis After Stent-Assisted Coiling of a Middle Cerebral Artery Aneurysm. [Journal Article]J Craniofac Surg. 2025 Dec 08. [Online ahead of print]JC
- CONCLUSIONS: Acute in-stent thrombosis is a severe complication of stent-assisted coil embolization. When strict indications are met, emergency intra-arterial thrombolysis is an effective rescue therapy. Perioperative intensive antiplatelet therapy, meticulous angiographic monitoring during the procedure, and early postoperative recognition coupled with prompt intra-arterial thrombolysis are central to achieving successful revascularization and improving clinical outcomes.

- Successful mechanical thrombectomy of a large venous graft thrombus burden using a novel device in an unusual way: a case report. [Case Reports]Eur Heart J Case Rep. 2026 Apr; 10(4):ytag170.EH
- Mechanical thrombectomy (mTE) is an established standard of care for selected patients with acute ischaemic stroke. However, in the cardiac equivalent-acute coronary syndrome-current guidelines do not recommend mTE. This is primarily based on trials showing no mortality benefit from routine thrombus aspiration, along with an increased risk of stroke.

- Tirofiban with sequential dual antiplatelet therapy in mild acute ischemic stroke (TiMIS): protocol for a multicenter, randomized controlled trial. [Journal Article]Ann Med. 2026 Dec; 58(1):2644698.AM
- Patients with mild acute ischemic stroke (AIS) (NIHSS score ≤5) remain at substantial risk for early neurological deterioration (END) and poor functional outcomes despite receiving standard dual antiplatelet therapy (DAPT). Tirofiban, a rapid-onset glycoprotein IIb/IIIa receptor inhibitor, may reduce this risk; however, evidence in patients with mild AIS is limited.
