- CONCLUSIONS: In this large symptomatic cohort, CEA and TCAR demonstrated similar perioperative safety, while TF-CAS was associated with higher 90-day stroke and 30-day mortality. One-year restenosis rates were low and comparable across all 3 approaches. These findings demonstrate that CEA and TCAR achieve comparably favorable outcomes with low rates of stroke, mortality, and perioperative complications, supporting both as safe and effective options for symptomatic patients. Conversely, TF-CAS was linked to significantly higher stroke risk, underscoring the need for selective use based on anatomy and patient-specific factors. Overall, the results reinforce a personalized, risk-adjusted approach to carotid revascularization.

This highlights the need for not only immediate workup but also close follow-up of those with IOEs, especially with these risk factors.

- CONCLUSIONS: Delayed CEA for symptomatic stenosis is usually due to doctor delay or patient delay. Medical reasons account for 19% of delayed operations. This implies that quality improvement is feasible by addressing doctor and patient delay. Healthcare providers should implement strategies to decrease the proportion of delayed CEA for symptomatic stenosis. Patient delay should be addressed with regular information campaigns.

- CONCLUSIONS: Patients undergoing CEA in the setting of amaurosis fugax had a statistically similar perioperative and 3-year stroke risk compared with asymptomatic patients. By contrast, hemispheric TIAs or stroke conferred greater risks longitudinally. These findings suggest that it may be appropriate to reconsider the historical risk perception associated with transient monocular blindness to a less severe phenotype and can inform preoperative decision-making for patients considering CEA.

- CONCLUSIONS: The high-altitude environment poses a serious threat to the cardiovascular systems of susceptible individuals.
