- Objective The aim of the German-language S3 guideline on lichen sclerosus (LS) is to provide an evidence- and consensus-based foundation for the diagnosis and management of this disease. The present publication offers a concise summary of the relevant content, focusing on genital LS in girls and women. This first part addresses, among other aspects, the definition, symptomatology, and diagnosis o…

- Treatment of Vulvar Lichen Sclerosus With Topical Ruxolitinib: A Single-Center Study. [Journal Article]J Dermatol. 2026 Aug 12. [Online ahead of print]JD
- Lichen sclerosus (LS) is a chronic inflammatory dermatosis of the anogenital region with limited therapeutic options. Emerging evidence implicates dysregulation of the Janus kinase (JAK)-signal transducer and activator of transcription (STAT) pathway in LS pathogenesis, supporting investigation of targeted therapies. We report a series of 6 patients treated with topical ruxolitinib for vulvar LS,…

- CONCLUSIONS: In this small retrospective series, ALA-PDT was associated with short-term clinical control and palliative benefit in older patients with extensive genital EMPD, while preserving genital anatomy without creating large surgical defects. These preliminary findings support further evaluation of optimized PDT-based conservative strategies for difficult-to-treat EMPD.

- Upadacitinib for refractory generalized lichen sclerosus: a case report and brief literature review. [Case Reports]Front Immunol. 2026; 17:1873990.FI
- Lichen sclerosus (LS) is a chronic, lymphocyte-mediated inflammatory skin disease characterized by porcelain-white atrophic skin lesions. Localized LS is relatively common and predominantly occurs in the anogenital region. Generalized LS involving both genital and extragenital sites is clinically rare and frequently refractory to conventional therapies like topical corticosteroids. As an emerging…

- Topical Tacrolimus for Lichen Sclerosus: Systematic Review of Efficacy and Safety. [Review]Dermatol Pract Concept. 2026 Jan 30; 16(1).DP
- CONCLUSIONS: Topical tacrolimus appears to be a safe and effective therapeutic option for lichen sclerosus, demonstrating favorable clinical and histological outcomes with minimal adverse effects. Further long-term prospective trials are needed to confirm its comparative efficacy, safety, and role in future treatment guidelines.

- Chronic vulvar itch: diagnostic and therapeutic challenges. [Review]Acta Dermatovenerol Alp Pannonica Adriat. 2025 Sep; 34(3):123-131.AD
- Vulvar pruritus refers to itching affecting the skin and mucosal surfaces of the external genital and perineal regions. It is most frequently associated with infections, inflammatory skin disorders, or neoplastic conditions. Due to the distinctive anatomical and physiological features of the anogenital area, clinical manifestations in this region are often subtle or atypical, which can complicate…
- Efficacy of Fractional CO2 Laser Therapy in Improving Symptoms and Quality of Life in Women with Refractory Vulvar Lichen Sclerosus: A Prospective Observational Study. [Journal Article]
- Lichen sclerosus (LS) is a chronic inflammatory condition predominantly affecting the anogenital region of postmenopausal women. It is associated with considerable aesthetic and functional impairments and an increased risk of squamous cell carcinoma. While high-potency topical corticosteroids remain the cornerstone of treatment, therapeutic options for patients with refractory LS are scarce. Frac…

- A Novel Nonsense Mutation of the ATP2C1 Gene in an 18-Year-Old-Female with Papular Acantholytic Dyskeratosis of the Anogenital Area. [Case Reports]
- Papular acantholytic dyskeratosis (PAD), often found to occur in the vulvar or anogenital area, is an exceedingly rare skin condition that usually presents in adulthood and features multiple smooth skin-colored or grayish-white papules with or without pruritus. Although the pathogenesis of PAD is unknown, PAD may be associated with mutations in ATP2C1 and ATP2A2 genes. Here, we report on an 18-ye…

- From Compression to Itch: Exploring the Link Between Nerve Compression and Neuropathic Pruritus. [Review]
- Neuropathic itch is a type of chronic pruritus resulting from neural dysfunction along the afferent pathway. It is often accompanied by abnormal sensations such as paresthesia, hyperesthesia, or hypoesthesia. This condition, which may involve motor or autonomic neural damage, significantly impacts patients' quality of life, causing severe itch and associated comorbidities such as depression, disr…

- Treatment of lichen sclerosus with hydroxychloroquine: a Mayo Clinic experience. [Journal Article]
- CONCLUSIONS: Hydroxychloroquine demonstrates promise as a therapeutic option for anogenital lichen sclerosus because of its favorable response rates and low incidence of adverse effects. However, further investigations, including larger-scale or prospective studies, are imperative to ascertain its definitive efficacy.

- A rare malignancy: A case report of early progression of anal Buschke-Löwenstein tumor into squamous cell carcinoma in an immunocompetent patient. [Case Reports]
- CONCLUSIONS: Although comprehensive research is lacking, wide local excision is considered the preferred first-line treatment for early-stage cases without evidence of local invasion. Furthermore, HPV immunization can prevent the development of Buschke-Löwenstein tumor, and early administration of the HPV vaccine is recommended to avoid acquiring HPV infection.

- Pinworm (Enterobius Vermicularis) Infestation: An Updated Review. [Review]
- CONCLUSIONS: In spite of effective treatment of pinworm infestation, recurrences are common. Recurrences are likely due to repeated cycles of reinfection (particularly, autoinfection) because of the short life span of adult pinworms. Good personal hygiene, such as frequent handwashing, especially after bowel movements and before meals, clipping of fingernails, avoidance of finger-sucking, nail-biting, and scratching in the anogenital area, are important preventive measures. Treatment of all household members should be considered, especially if there are multiple or repeated symptomatic infections.

- Peri-clitoral Epidermal Inclusion Cyst as Initial Presentation of Lichen Sclerosus in a Pediatric Patient. [Case Reports]
- CONCLUSIONS: Thorough workup of clitoromegaly negative for hormonal causes requires further investigation to determine an alternative etiology of the mass. We suspect that inflammatory changes of LS and pruritus resulted in the peri-clitoral inclusion cyst.
