- Hypertransaminasemia in hospitalized children: Insights from a National Multicenter Study by the Italian Society of Pediatric Gastroenterology, Hepatology, and Nutrition. [Journal Article]J Pediatr Gastroenterol Nutr. 2026 Aug 18. [Online ahead of print]JP
- CONCLUSIONS: Hypertransaminasemia is common in children admitted to the general pediatric units, affecting approximately 35% of patients. Liver-related discharge diagnoses were uncommon among children with elevated transaminases (5.70%), whereas most cases occurred in the setting of acute non-hepatic illnesses. Children with persistently elevated transaminases at discharge should be followed to rule out chronic liver disease.

- Regional, altitudinal, and age-specific heterogeneity of bronchiolitis hospitalization seasonality in Ecuador, 2007-2024, and implications for RSV immunoprophylaxis timing. [Journal Article]
- CONCLUSIONS: Bronchiolitis seasonality in Ecuador changed after the pandemic, most clearly and stably in Costa; Sierra showed a transient 2022 perturbation with partial reversion by 2023-2024, and Amazonía's estimates remained unstable and underpowered. As region-tailored estimates are less stable in Sierra and Amazonía than in Costa, a single national window (nirsevimab February-April; maternal RSVpreF for pregnancies expected to deliver January-May) is a more defensible starting point, pending confirmation from further seasons and, ideally, laboratory-confirmed surveillance.

- Misdiagnosis of bronchiolitis and misuse of antibiotics - experience from the largest paediatric hospital of Bangladesh. [Journal Article]
- CONCLUSIONS: There was a high proportion of bronchiolitis among WHO-classified pneumonia, and antimicrobials were prescribed to treat those cases. The findings need the attention of policymakers to address the overlap between WHO-classified pneumonia and bronchiolitis and minimise the potential overuse of antibiotics.

- Descriptive Analysis of the First 12-Month Activity of Docadom, a Private Physician-Led Urgent In-Home Consultation Service. [Journal Article]
- Background: Emergency services in urban areas are overcrowded due to a rise in mild-to-moderate acuity consultations, and physician-led home visits are less common. In response, a home-based urgent care model in an urban setting (Docadom) was developed. Consultation requests via a dedicated mobile application or phone call are triaged by a trained emergency nurse. Physicians travel to the patient…

- Advancing equity: six-year respiratory outcomes in young Indigenous children following early respiratory disease detection. [Journal Article]Respir Med. 2026 Aug 12; 262:109103. [Online ahead of print]RM
- CONCLUSIONS: In this first whole-available community longitudinal study of remote-based Indigenous children, there was high prevalence of chronic respiratory disease with corresponding low lung function. CWC in the first eight-years of life and previous ALRI-related hospitalisation were associated with significantly increased risk of CSLD/bronchiectasis.

- Exposure to Airborne Particulate Matter in Regional Environments of Bangladesh and Its Effects on Respiratory Health: A Narrative Review. [Journal Article]Health Sci Rep. 2026 Aug; 9(8):e72925.HS
- CONCLUSIONS: The findings of the present review indicates that the urgent regulatory enforcement, enhanced air quality monitoring, and targeted public health interventions are essential to mitigate respiratory health risks and reduce the nationwide burden of air pollution in Bangladesh. Furthermore, comprehensive geological and environmental monitoring studies are required across all districts to fully characterize exposure and guide effective public health interventions.

- Severe Pulmonary Cryptococcosis in an Immunocompetent Patient Presenting With Acute Respiratory Failure. [Case Reports]Cureus. 2026 Jun; 18(6):e111726.C
- Pulmonary cryptococcosis may present with nonspecific respiratory symptoms and, in patients with underlying reactive airway disease, can act as a trigger for refractory asthma exacerbation leading to acute respiratory failure, even in immunocompetent individuals. A 60-year-old immunocompetent male, with a negative HIV test, no chronic immunosuppressive therapy, and a remote brief corticosteroid c…

- Focus on Rare Tracheal Pathologies You Should Not Miss. [Case Reports]J Clin Med. 2026 Jul 13; 15(14).JC
- Backgroud: Pathologies of the trachea and large bronchi are a heterogeneous group of rare structural disorders of the airway wall that are frequently underdiagnosed because their symptoms overlap with common respiratory diseases. Methods: We present three patients with chronic cough and dyspnea lasting approximately three years who had previously received multiple incorrect diagnoses, including C…

- Extracorporeal life support for severe heart failure in a diffuse panbronchiolitis patient awaiting lung transplant: a case report. [Case Reports]
- A 55-year-old woman with chronic respiratory failure due to diffuse panbronchiolitis, on a lung transplant waitlist, developed acute left ventricular failure with an ejection fraction of 18%. Although cardiac reversibility was uncertain, it was considered potentially reversible and transplant eligibility was judged to be maintainable; therefore, veno-arterial extracorporeal membrane oxygenation w…

- Shi-Wei-Long-Dan-Hua capsule (SWLDH) is clinically used to alleviate bacterial or viral infection induced bronchitis. Recent studies have reported its effects on viral infection induced acute lung injury, broadening our understanding of its clinical applications. However, its effects against bacterial infection-induced ALI were still unclear, along with not fully elucidated chemical profile, sign…

- Acute exacerbation in fibrotic interstitial lung disease: An International Working Group Report. [Journal Article]Am J Respir Crit Care Med. 2026 Jul 14. [Online ahead of print]AJ
- Acute exacerbations (AEs) occur both in patients with idiopathic pulmonary fibrosis (IPF) and non-IPF fibrotic interstitial lung disease (fILD). These events confer high morbidity and mortality, with a lack of proven effective therapeutic interventions. The objective of this state-of-the-art document is to summarize latest evidence since the 2016 international working group report on AE-IPF, expa…

- Efficacy and safety of inhaled ambroxol hydrochloride solution in Chinese pediatric patients with acute lower respiratory tract infections: a real-world, multicenter, open-label, single-arm study. [Journal Article]Transl Pediatr. 2026 Jun 30; 15(6):218.TP
- CONCLUSIONS: This large real-world study demonstrated that a 7-day course of AHSI, added to standard care, was associated with clinically meaningful improvements in respiratory symptoms and a favorable safety profile in children with ALRTI. The consistent effects across disease subtypes and nebulizer devices underscore the practical utility of AHSI in diverse pediatric settings. While the single-arm design limits causal inference, these findings provide robust real-world evidence supporting AHSI as an effective expectorant option. Prospective confirmation through RCTs will further define its role in first-line therapy.
