  • CONCLUSIONS: This case highlights the diagnostic challenges and clinical relevance of internal carotid artery fenestration, particularly in patients with systemic vascular risk factors. Although internal carotid artery fenestration is often asymptomatic, it can be associated with cerebrovascular complications, such as ischemic events. In this case, the transient symptoms likely resulted from localized hemodynamic disturbances due to the fenestrated artery. While there is no established consensus on the management of asymptomatic internal carotid artery fenestration, dual antiplatelet therapy and risk factor optimization remain key strategies. Further research is needed to better understand the implications of internal carotid artery fenestration and to refine diagnostic and management protocols for these rare vascular anomalies.

  • CONCLUSIONS: The significant overlap in symptoms of orbital cellulitis and CCF complicates diagnosis. When two diseases coexist, the diagnosis becomes more complex. This rare case report highlights that CCF should be considered a potential concurrent pathology in patients with head and neck trauma presenting with orbital cellulitis. In addition, CCF can be considered in trauma patients exhibiting dehiscent ICA, irrespective of orbital symptoms, and prior to performing neuroimaging.

  • CONCLUSIONS: It is appropriate to use this technique in patients with adequate inflow and outflow, but with focal vein stenoses or short, thickened vein segments that preclude the use of a standard AVF. This hybrid approach, combining principles of patch venoplasty and bovine pericardial bypass grafting, may be used as a salvage option in selected patients with focal venous stenotic lesions considered for native arteriovenous fistula creation.

Transvenous embolization via the cervical epidural venous system offers a safe and effective route for definitive fistula occlusion without neurological dysfunction.
