  • CONCLUSIONS: GC and conventional IS currently remain the most evidence-based approaches for managing most cardiopulmonary manifestations in SLE. Despite aggressive immunosuppressive therapies, patients with some conditions, such as LM, ALP, and DAH, still experience high mortality. The quality of the existing evidence and the insights into novel therapies in these patients were limited, highlighting the urgent need for RCTs and optimized management in this area.

  • CONCLUSIONS: This case illustrates the diagnostic challenges and consequences of a cardiac tumor which was not apparent on serial echocardiograms.Primary cardiac malignancy should be considered in the setting of recurrent pericardial effusion with systemic symptoms and persistently elevated inflammatory markers. Echocardiogram has shown low sensitivity for cardiac tumors in the setting of effusion; advanced imaging modalities can facilitate early detection and intervention.

  • CONCLUSIONS: Severe primary hypothyroidism should be routinely considered in patients with unexplained pericardial effusion, even in tuberculosis-endemic settings. Paradoxical bradycardia in tamponade, Woltman's sign, low-voltage electrocardiography, and a protein-rich pericardial effusion with low adenosine deaminase are important diagnostic clues. Prompt pericardiocentesis treats hemodynamic compromise, while cautious levothyroxine replacement prevents recurrence.

  • CONCLUSIONS: In this exploratory analysis, US-GPPCD showed acceptable short-term safety under direct supervision. A significant inflection point in procedural efficiency was observed at approximately 14 cases, while operator confidence consolidated shortly thereafter. These findings provide preliminary, hypothesis-generating evidence that may inform future training protocols, but require validation across multiple operators and centers.
