- CONCLUSIONS: Focal and diffuse adenomyomectomy represent two distinct surgical approaches reflecting different patterns of adenomyotic involvement and operative complexity. Focal adenomyomectomy is more often amenable to minimally invasive techniques and targeted excision of localized lesions. In contrast, diffuse adenomyomectomy is generally associated with more extensive surgery and complex uterine reconstruction and may be associated with greater symptom improvement and favorable reproductive outcomes, based on reported outcomes across studies. Despite differences in surgical complexity, obstetric outcomes following conception appear largely comparable between groups.

- CONCLUSIONS: Multiple biopsychosocial factors influence sexual activity and function in women with PFD. Approximately 2 out of 5 women were sexually inactive and a quarter were at risk of sexual dysfunction. - CONCLUSIONS: Fractional carbon dioxide laser therapy was associated with significant symptom improvement at short-term follow-up after treatment completion in this clinical cohort. No statistically significant additional benefit was observed with concomitant topical vaginal oestrogen, although confounding by indication cannot be excluded and the study had limited power. Larger randomised sham-controlled trials with longer follow-up are required to determine comparative efficacy and durability of response.

- CONCLUSIONS: Although severe dysmenorrhea and long duration of symptoms were associated with SPE, symptom profiles and functional questionnaires were insufficient for accurate prediction. The calculated score performed modestly internally but failed external validation. Given that symptoms common to endometriosis are also reported in women with other etiologies of pelvic pain, symptom-based models appear inadequate for reliable identification of SPE. Integrating clinical factors with biomarkers or emerging diagnostic technologies may be necessary to improve non-invasive diagnostic accuracy. Clinicians and patients should be aware of the high rate of negative surgical findings when making the decision to proceed with laparoscopy versus considering conservative medical therapies instead.
