- CONCLUSIONS: Real-world use of GLP-1 RAs in AKD remains modest but is accelerating, especially in patients with T2DM, CKD stage 3 and obesity. Given the elevated rates of hypoglycemia, mood disorders and gastrointestinal intolerance, careful monitoring is warranted when prescribing these agents in the post-dialysis AKD setting.

- Prevalence and clinical determinants of diabetic gastroparesis in type 2 diabetes: a symptom- and scintigraphy-based cross-sectional study. [Journal Article]
- CONCLUSIONS: DG appears to be a clinically relevant but potentially underrecognized complication of T2D. Poor glycemic control, obesity, and longer duration of diabetes are associated with delayed gastric emptying. Limited agreement between symptom severity and objectively measured gastric emptying supports the continued role of gastric emptying scintigraphy as the diagnostic reference standard.

- CONCLUSIONS: This case underscores a previously unreported but serious complication of GLP-1 RA therapy, transmural esophageal rupture, likely precipitated by drug induced gastroparesis and forceful emesis. Restarting semaglutide at a high dose without titration after a prolonged interruption likely increased vulnerability to injury. Clinicians should maintain a high index of suspicion for esophageal complications in patients presenting with chest pain and vomiting during GLP-1 RA initiation or reinitiation. Early multidisciplinary management is crucial to optimizing outcomes in this rare but life-threatening scenario.
