- Current Evidence on the Use of Biomarkers for the Diagnosis of Acute Mesenteric Ischemia: A Narrative Review. [Review]Cureus. 2026 Jul; 18(7):e112865.C
- Acute mesenteric ischemia (AMI) is a life-threatening vascular emergency associated with high morbidity and mortality, largely because diagnosis is frequently delayed by non-specific clinical manifestations. Although computed tomography angiography (CTA) remains the diagnostic gold standard, considerable research has focused on identifying laboratory biomarkers that may facilitate earlier recogni…

- Hybrid Endoscopic-Laparoscopic Management of Colonoscopic Perforation within an Inguinal Hernia Sac. [Case Reports]J Vis Exp. 2026 Jul 24; (233).JV
- Colonoscopy is the gold standard for colorectal cancer screening, yet it carries the risk of iatrogenic perforation. A rare, hazardous subtype involves colonoscope incarceration and perforation within an inguinal hernia sac, mechanically driven by a "hinge effect" during withdrawal. The confined anatomical space often masks typical peritoneal signs, challenging diagnosis. A case is presented of a…

- A Bedside Prediction Score for Strangulation in Adults With Acute Mechanical Intestinal Obstruction: A Prospective Cohort Study in Taiz, Yemen. [Journal Article]Cureus. 2026 Jul; 18(7):e112348.C
- Background Early recognition of bowel strangulation in acute mechanical intestinal obstruction (AMIO) is critical but remains difficult when computed tomography (CT) is unavailable, delayed, or not immediately actionable. This study aimed to develop and internally validate a CT-independent bedside prediction score for estimating the risk of bowel strangulation in adults with AMIO in Taiz, Yemen. …

- Long-term antithrombotic therapy in patients with short bowel syndrome: A European Society for Clinical Nutrition and Metabolism (ESPEN) position paper. [Journal Article]Clin Nutr. 2026 Jul 28; 64:106741. [Online ahead of print]CN
- CONCLUSIONS: While robust evidence is lacking regarding antithrombotic therapy in patients with SBS, consensus was reached on the majority of statements. More research in this field is required, particularly on oral drug absorption in SBS, development of alternative oral drug formulations, treatment strategies following arterial mesenteric stenting and direct oral anticoagulant (DOAC) dosing for secondary prophylaxis of CRT in SBS.

- Therapeutic Effects of Carvedilol on Intestinal Mucosa in Intestinal Ischemia Reperfusion Injury Model; An Experimental Study. [Journal Article]J Indian Assoc Pediatr Surg. 2026 Jul-Aug; 31(4):630-637.JI
- CONCLUSIONS: Postreperfusion administration of carvedilol significantly attenuated intestinal I/R injury, as evidenced by reduced histopathological damage scores and preserved thiol/disulfide homeostasis. These findings suggest that carvedilol's antioxidant properties may offer therapeutic potential in intestinal I/R injury. To our knowledge, this is the first study evaluating carvedilol in this context, warranting further investigation with different dosing regimens and timepoints.

- Inflammatory gut-liver crosstalk: mechanisms and therapeutic targets. [Review]
- The gut has a profound influence on the liver through their anatomical connection via the portal vein. During acute inflammation, gut tissue injury leads to increased barrier permeability, allowing the translocation of external contents that can affect hepatic function. Gut-liver crosstalk contributes to the pathophysiology of acute inflammatory disorders, such as sepsis, intestinal ischaemia-rep…

- Meckel's diverticulum band adhesion causing internal hernia, small bowel obstruction complicated by peritonitis and massive ascites: a rare pediatric case. [Journal Article]Front Pediatr. 2026; 14:1834207.FP
- Meckel's diverticulum (MD) is one of the most common congenital anomalies of the gastrointestinal tract in children, although it may occasionally be detected in adults. It is typically located within 100 cm of the ileocecal valve in the distal ileum. The diverticulum varies in size and morphology; many patients remain asymptomatic throughout life. In children, the most common clinical presentatio…

- Clinical features of failure of conservative management in adult patients with adhesive small bowel obstruction: a retrospective cohort study. [Journal Article]Front Med (Lausanne). 2026; 13:1870352.FM
- CONCLUSIONS: Our study established older age, higher CRP level, fever, and larger MDSB on admission as risk factors for failure of conservative ASBO management. The risk of ASBO recurrence was not increased by surgical intervention. Although these results should be interpreted with caution, these findings might assist clinical practice. Conversion to operation increases the length of hospital stay, hospitalization costs, and intensive care unit admission.

- Surgical abdomen masked by metabolic crisis: fulminant ischemic colitis in severe diabetic ketoacidosis - a case report and literature review. [Case Reports]Front Endocrinol (Lausanne). 2026; 17:1875153.FE
- CONCLUSIONS: This case highlights a critical diagnostic challenge: apparent biochemical improvement during a hyperglycemic crisis, such as the narrowing of the anion gap, does not exclude evolving intestinal ischemia. Endocrinologists and critical care physicians must recognize that disproportionate or progressive abdominal signs require prompt imaging and early surgical consultation, even when contrast-enhanced studies are contraindicated.
