- CONCLUSIONS: PLAM is a rare lung disease characterized primarily by diffuse interstitial lung lesions. It poses challenges in early diagnosis and has a poor prognosis. Its relationship with TB is complex, as the two conditions mutually promote each other and can act as both cause and effect. When the two coexist, the clinical manifestations are diverse. In-depth exploration of their pathological characteristics, diagnosis, and differential diagnosis is conducive to clinical diagnosis and treatment.

- CONCLUSIONS: We describe two AS patients with bilateral upper-lobe destruction whose initial chest CT scans suggested tuberculosis. After thorough microbiological work-up remained negative, both were diagnosed with AS-associated lung injury (ASLI) and referred to rheumatology. Because destroyed lung carries lifelong risks of infection and hemorrhage, long-term respiratory follow-up was recommended.

- CONCLUSIONS: This case documents an unusual presentation of localized pleural dissemination without pathologically confirmed nodal involvement in a central ALK-rearranged lung adenocarcinoma. It highlights the importance of intraoperative pathological confirmation, accurate staging, and timely molecular testing. However, mechanistic and therapeutic implications should be interpreted cautiously in light of the single-case nature of this report.

- CONCLUSIONS: This case report suggests that for patients with complex pulmonary aspergilloma and non-tuberculous mycobacterial lung infection who are ineligible for surgery and cannot tolerate high-dose systemic antifungal therapy, bronchoscopic lesion resection combined with endobronchial instillation therapy may represent a feasible treatment option with potential clinical benefits. It also serves as a reminder to remain vigilant for the occurrence of aspergillosis following non-tuberculous mycobacterial lung disease.

- CONCLUSIONS: This case adds to the limited literature on Mycobacterium shimoidei pulmonary disease and supports its role as a clinically relevant cause of cavitary nontuberculous mycobacterial infection. Severe anorexia nervosa may represent a predisposing condition as it might be associated with structural lung diseases. The case further underscores the importance of next-generation sequencing for the identification of rare NTM species.
