  • CONCLUSIONS: PLAM is a rare lung disease characterized primarily by diffuse interstitial lung lesions. It poses challenges in early diagnosis and has a poor prognosis. Its relationship with TB is complex, as the two conditions mutually promote each other and can act as both cause and effect. When the two coexist, the clinical manifestations are diverse. In-depth exploration of their pathological characteristics, diagnosis, and differential diagnosis is conducive to clinical diagnosis and treatment.

  • CONCLUSIONS: We describe two AS patients with bilateral upper-lobe destruction whose initial chest CT scans suggested tuberculosis. After thorough microbiological work-up remained negative, both were diagnosed with AS-associated lung injury (ASLI) and referred to rheumatology. Because destroyed lung carries lifelong risks of infection and hemorrhage, long-term respiratory follow-up was recommended.

  • CONCLUSIONS: This case demonstrates that invasive pulmonary fungal infection can perfectly mimic the typical radiographic features of hematogenous disseminated pulmonary tuberculosis, including diffuse small nodular and subsolid lesions with a miliary distribution pattern predominantly in the upper lobes and extrapulmonary manifestations such as erythema nodosum. For unexplained pulmonary infections in immunocompromised hosts where conventional diagnosis and empirical treatment fail, the timely application of bronchoscopy combined with mNGS technology represents a critical breakthrough for achieving precise diagnosis.

  • CONCLUSIONS: This case demonstrates the complexity of multi-stage treatment, including chemotherapy, surgery, anti-infection and immunotherapy, for a patient with LSCC of the right lobe, accompanied by multiple complications. Bronchial stent implantation and adjustment significantly improved the airway obstruction, while the precise anti-infection strategy based on etiology effectively controlled the infection. Consideration of factors such as the patient's smoking history had an important impact on the development of the disease and the therapeutic effect.
