- Eletriptan-Associated Coronary Vasospasm in a Heart Transplant Recipient. [Case Reports]JACC Case Rep. 2026 Aug 18; :109808. [Online ahead of print]JC
- CONCLUSIONS: This case demonstrates acute drug-induced coronary vasospasm following eletriptan use in a heart transplant recipient and highlights the importance of recognizing reversible vasospasm before escalating coronary intervention.Drug-induced coronary vasospasm should be considered in heart transplant recipients with acute coronary syndrome after exposure to vasoconstrictive medications. Recognition of reversible vasospasm may prevent unnecessary coronary intervention and inappropriate escalation of vasculopathy management.

- The impact of parathyroid hormone replacement with palopegteriparatide on synthesis of 1,25-dihydroxy vitamin D in hypoparathyroidism. [Journal Article]Expert Rev Clin Pharmacol. 2026 Aug 16; :1-11. [Online ahead of print]ER
- Parathyroid hormone (PTH) is the primary regulator of calcium and phosphate balance via direct action on the kidneys and bones and indirect action on the intestine. Hypoparathyroidism is caused by insufficient levels of PTH, leading to 1,25-dihydroxy vitamin D (1,25[OH]2D) deficiency. PTH-induced 1,25(OH)2D production is a highly regulated process essential for physiological functions that is red…

- Toe walking: A regional analysis of TTN variants between c.10,000 and c.99,999 in a cohort of children. [Journal Article]
- CONCLUSIONS: Children carrying TTN variants within the c.10,000-99,999 region exhibited broadly similar clinical phenotypes characterized by persistent toe walking and reduced ankle dorsiflexion. Stratification by cDNA location did not reveal clear genotype-phenotype clustering, suggesting limited predictive value of variant position alone and supporting the need for functional and segregation-based studies in future research.

- Management of Generalized Adult Tetanus with Intermittent Intrathecal Baclofen Bolus: Overcoming Infrastructural Limitations in a Resource-Limited Setting. [Case Reports]Am J Trop Med Hyg. 2026 Aug 13. [Online ahead of print]AJ
- Generalized tetanus presents a formidable therapeutic challenge, especially when refractory to conventional management. Standard treatment involves toxin neutralization, muscle spasm control (sedation and magnesium sulfate), and supportive care. However, refractory cases often necessitate prolonged intensive care and risk secondary complications. Intrathecal baclofen (ITB), a gamma-aminobutyric a…

- CONCLUSIONS: Both nabiximols and cannabis decoction are effective for MS-related spasticity. Cannabis decoction may represent a viable option for patients who do not respond to or tolerate nabiximols. Differences in cannabinoid composition and pharmacokinetics may explain variations in outcomes. Prospective studies are needed to confirm these findings and optimize treatment strategies.

- Development of a nomogram for predicting severe post-parathyroidectomy hypocalcemia in patients with secondary hyperparathyroidism: a retrospective cohort study. [Journal Article]Gland Surg. 2026 Jul 31; 15(7):192.GS
- CONCLUSIONS: We developed and internally validated a nomogram for preoperative risk stratification of SPPH in patients receiving intensive calcium supplementation protocols using readily available preoperative variables, identifying ALP as the predominant risk factor. The prognostic significance of ALP offers a new perspective beyond the conventional emphasis on PTH levels and insight into the underlying mechanisms of post-operative hypocalcemia. The substantial gradient across risk groups (8.0-72.5%) supports personalized perioperative management. However, multicenter external validation is essential before clinical implementation. Once validated, this tool could enhance patient outcomes and streamline healthcare resources for thousands of patients undergoing parathyroidectomy annually.

- Pre-Dialysis Serum Myostatin Concentration Is Independently Associated with Intradialytic Muscle Cramps in Hemodialysis Patients. [Journal Article]
- Muscle cramps are among the most common acute complications of hemodialysis (HD), often occurring even in the absence of electrolyte disturbances. Most cramps are painful and may cause patients to fear future dialysis sessions. We hypothesized that susceptibility to muscle cramps may be related to impaired skeletal muscle myokine secretion. No significant differences in pre-dialysis myokine level…
