Total knee arthroplasty (TKA) after periarticular fracture fixation, malunion, high tibial osteotomy, or other previous knee surgery differs substantially from routine primary TKA. Retained implants, extra-articular deformity, distorted landmarks, bone defects, scarred soft tissues, and infection risk may all compromise standard instrumentation and implant positioning. Robotic-assisted TKA (RA-TKA) is increasingly used in selected non-standard primary cases because computed tomography -based or imageless planning can define deformity, anticipate implant-hardware conflicts, guide bone resection, and quantify gap balance. However, the available evidence remains limited and heterogeneous, consisting mainly of case reports, small case series, and early retrospective comparative cohorts. This scoping review synthesizes clinical evidence on RA-TKA in post-traumatic and previously operated knees with retained or recently removed hardware. It also integrates relevant navigation and hardware-removal literature to propose a practical workflow for patient selection, imaging, hardware strategy, robotic execution, implant escalation, intraoperative safety checks, and implementation. Current literature supports technical feasibility and suggests potential workflow advantages in carefully selected cases, particularly when landmarks are unreliable, intramedullary instrumentation is blocked, or three-dimensional planning may clarify implant-hardware conflict. These findings should not be interpreted as evidence of proven superiority. Comparative evidence remains sparse, and follow-up is short. Current data do not establish improved survivorship, lower infection risk, better patient-reported outcomes, or cost-effectiveness. RA-TKA should therefore be framed as a planning and execution adjunct, not as a stand-alone solution for complex conversion-style primary TKA.

Germline genetic testing plays a critical role in diagnosing inherited predispositions and increasingly guides therapeutic and surveillance choices-but becomes technically challenging after allogeneic hematopoietic stem cell transplantation (HSCT), when donor-derived DNA contaminates host tissues. To address this, we compared donor-derived DNA across three accessible tissues-buccal swab, nail, and eyebrow follicles-in recipients after hematopoietic stem cell transplantation using two orthogonal assays (34-SNP next-generation sequencing and a 27-marker short tandem repeat panel) and modeled clinical covariates that influence chimerism. Eyebrow follicles showed consistently low donor DNA (median 1% by NGS; 3% by STR) whereas buccal swabs and nails carried substantially higher donor fractions (+25 and +22 percentage points versus eyebrow, respectively; both p < 0.01). Across methods, STR yielded on average ≈6 percentage points higher donor fractions than NGS at low-level chimerism. Several transplant covariates correlated with chimerism: matched-related donors and a perfect HLA match (10/10) were each associated with lower donor DNA (≈12-14 and 15-20 percentage points, respectively); longer times since hematopoietic stem cell transplantation correlated with lower levels for nail samples, and donor-recipient sex match correlated with higher donor DNA (~7-8 percentage points). Even low-level chimerism can distort germline variant interpretation. We propose a pragmatic protocol for post-hematopoietic stem cell transplantation germline testing that prioritizes eyebrow follicles as the default tissue. An SNP-based quality control assay is used to flag unsafe donor fractions (≥ 5-10%) before comprehensive germline analysis, reducing the risk that chimeric donor DNA distorts germline variant interpretation.

Developmental dysplasia of the hip (DDH) is a serious condition resulting in inadequate acetabular development, distorted bone configuration, and substantially altered hip biomechanics. An extensive leg length discrepancy (LLD) is commonly encountered in such cases, making a total hip arthroplasty (THA) procedure extremely challenging. Although good results in terms of patients' satisfaction, implant survival rates and overall improved quality of life have been reported, complication rates are considerably higher than primary THA procedures performed for idiopathic osteoarthritis. Reconstructing a dysplastic hip arthrosis and equalizing a preexisting LLD is a technically demanding procedure that is associated with significant bone and soft tissue complications. Intramedullary lengthening through motorized nails has become increasingly popular to address difficult cases with extensive LLD following THA in recent years. However, limited data on femoral lengthening procedures implemented following THA are available considering complications, radiological results, and patient-reported outcomes following staged THA and subsequent femoral lengthening using a femoral magnetically-driven intramedullary lengthening nail. We performed a literature review of the past 10 years in PubMed using the terms neglected hip dislocation, DDH, THA, and intramedullary lengthening nail as keywords. A total amount of eight cases addressing LLD through a telescoping intramedullary nail following THA in DDH have been reported in recent literature. All eight patients underwent primary THA for DDH followed by the implantation of the intramedullary lengthening nail. The mean THA was lengthened by 28.9 mm (from 13.0 to 45.0). The mean time for nail implantation after THA was 11.1 months (from 3.5 to 21). The mean time for lengthening per day through the nail was 0.94 mm (from 0.65 to 1.0) from 26 days to 70 days, and the mean lengthening through the nail was 37.6 mm (from 24.0 to 70.0). Good union and consolidation rates were reported by the authors, while there were no complications. The intramedullary distraction osteogenesis method with a telescopic rod can be an effective method to manage leg length discrepancies while avoiding soft tissue complications in challenging cases of DDH.

Body representation distortion (BRD) is a core criterion of Anorexia Nervosa (AN), and is usually assessed subjectively, focusing on body shape. We aimed to develop a new assessment to evaluate body representation independently from socially-mediated body image, on a body part with low emotional salience (hands). In a monocentric open label pilot study, we measured hand representations based on explicit (verbal) and implicit (tactile) instructions. Participants, with eyes closed, had to point targeted locations (knuckles and nails of each finger) based on verbal instructions and tactile stimulations to evaluate body representations respectively. Ratios between hand width and finger length were compared between AN (n = 31) and controls (n = 31) and correlated with current body mass index, AN subtype and disease duration. To control that hand distortion was specific to body representation, we also assessed object representation. Hand representation's width/length ratio was significantly increased in patients with AN, whereas no difference was found in object representation. We found no correlation between hand wideness and clinical traits related to eating disorders. Our results propose that BRD is not limited to body parts with high emotional salience, strengthening the hypothesis that anorexia nervosa is associated with profound unspecific BRD.

Fingertip injuries result in significant morbidity. It is associated with pain, loss of work and recreational hours, financial burden, and distortion of body image. Although it is a common injury, the epidemiology of fingertip injury is a subject with scanty literature. This study is aimed at providing epidemiology of fingertip injury in India. Epidemiological data were collected and analysed through a retrospective review of all fingertip injuries over a period of four years treated in our department. In the paediatric population, among the 241 injuries in 221children, most occurred at home (98%). Most incidents were in children under five years of age (74%). The most commonly involved digit is the middle finger (29%) and door crush was the commonest reason (80%). Incomplete fingertip amputation with nail bed injury was seen in 80% of cases. In adults, there were 351 injuries in 290 patients, most of which occurred by jamming of the finger at a two-wheeler chain (22.5%). The second most common cause is cut by machine and associated with heavy machinery (17.6%). Complete amputation at the nailbed level was seen in 50% of the cases. Both children and adults need to be educated regarding the causation and effects of fingertip injuries. Damage to fingers can be prevented and reduced by observing safety measures both inside the home and at the workplace.

In bone fracture healing, new tissue gradually forms, ossifies, and eventually remodels itself to restore mechanical stiffness and strength across injury site. Mechanical strain at the fracture site has been implicated in controlling the process of healing and numerical mechanoregulation models with strain-based fuzzy logic rules have been applied to simulate bone healing for simple fracture geometries. However, many of these simplified models cannot capture in vivo observations such as delays in healing with torsional instability or differences in healing rate between different fracture types. Accordingly, the purpose of this work was to apply a fuzzy logic mechanoregulation fracture healing simulation technique to 3D models representing a range of clinically inspired fracture geometries with intramedullary nail fixation and multiaxial loading conditions. The models predicted that the rate of healing depends on the geometry of the fracture and that all fracture types experience a small healing delay with torsional instability. The results also indicated that when realistic torsional loading and fixator mechanics are included, previously published strain-based rules for tissue destruction lead to simulated nonunions that would not be expected in vivo. This suggested that fracture healing may be more robust to distortional strain than has been previously reported and that fuzzy logic models may require parameter tuning to correctly capture clinically relevant healing. The strengths of this study are that it includes fracture morphology effects, realistic implant mechanics, and an exploratory adaptation of the upper distortional strain threshold. These findings may help future researchers extend these methods into clinical fracture healing prediction.

In digital replantation, Kirschner wire (K-wire) fixation has commonly been used for osteosynthesis. On the other hand, K-wires are often obtrusive because of protrusion from the replanted digit. We describe a case series treated using hydroxyapatite/poly-L-lactide (HPLLA) threaded pins, which are not only bioabsorbable, but also osteoconductive, for osteosynthesis in crushed amputation or comminuted fracture, including distal phalanx amputation. Using an HPLLA threaded pin, 10 digital replantations were performed between July 2016 and April 2018. The precisely cut pin is manually pushed into the fracture site as an intramedullary nail. The pin is first pushed into the distal site, and then into the proximal site, after drilling with a K-wire of the same diameter as the pin. All amputations were crush type, and levels of amputations were Tamai zone I in three cases, zone II in two and zone III in five. Eight of the 10 digits survived. The two digits that did not survive showed venous insufficiency. Bone union of the eight digits was successfully obtained after 9-19 weeks (median 12 weeks). No adverse events occurred such as distortion of the pin, infection or foreign body reaction. The HPLLA threaded pin may be an adequate device to fix fractures in digital replantation.

A 60-year-old patient suffered an ankle distortion resulting in a comminuted fracture of the fifth metatarsal. On the same day the accident occurred the patient presented to an emergency department and immediate operative treatment of the fracture was performed by intramedullary Kirschner wires with closed reduction and internal fixation. The aftercare was carried out on an outpatient basis by a registered orthopedist in accordance with the surgeon's instructions. In the radiological control 14 days after surgery a dislocation of the fracture was detected and 1 week later a rectification operation was carried out in a second clinic with removal of the Kirschner wires, open reduction and internal fixation with a locking plate. The patient suspected inadequate treatment by the first clinic, which in turn did not accept any inadequacies in the surgical treatment or the aftercare. In the subsequent legal dispute the appointed experts came to the conclusion that an initial good fracture position was achieved but that the aftercare treatment with a forefoot relief shoe was inadequate for the type of internal fixation chosen. The arbitration board came to the conclusion that the multifragmented fracture situation was treated by a questionably stable osteosynthesis using Kirschner wires. This should have required a very strict aftercare with partial weight bearing on crutches and immobilization in a stable orthosis or cast.

PURPOSE OF THE STUDY The calcaneal bone is affected in 2% of body fractures. Because of its major influence to walking stability with its permanent consequences affecting both daily living and work activities, huge effort is expended on suitable treatment of these fractures. Many studies have focused on surgical treatment of calcaneal fractures or on comparing operative and non-operative treatment. The aim of this retrospective study is to investigate stability of different type of osteosynthesis. MATERIAL AND METHODS This study involves 119 men and 24 women aged 20 to 84 years who were in the period from 2011 to 2015 surgically treated for broken calcaneus in the Trauma Hospital in Brno - this study covers 152 calcaneal fractures in 143 patients. The calcaneal fractures were classified into Sanders I-IV subgroups based on the CT scans and divided by the applied type of osteosynthesis. The Böhler and Gissane angle as well as height, width and length of calcaneal bone at the time after reposition and stabilization and at 3 and 12 months after the trauma were compared. Also tracked was the relationship between the imaging scans and the clinical assessment based on the AOFAS Ankle-Hindfoot Scale (A-H score). RESULTS Decrease of the Bohler angle (increment of the Gissane angle) of more than 5 degrees and reduction of height and extension of length or width of the calcaneal bone of more than 2 mm were defined as a criterion of osteosynthesis failure. These conditions were met in 53 surgically treated calcaneal fractures (35% of the total). In 17 cases the angular stable plate failed (it is 36% of the used plates), in 21 cases it was the C-nails (42%) and in 11 cases the screws (24%) that failed. In this group, 20 patients (38%) were satisfied with 100 points in the A-H score, 16 patients (30%) felt good, 11 patients (21%) quite good and 5 patients (9%) felt bad. DISCUSSION It is difficult to compare the stability of different types of osteosynthesis in a retrospective study because of the compliance impossibility, differences in the follow-up time and distortion of X-rays. The results are distorted in uncooperative patients, smokers, alcohol abusers and in elderly patients. CONCLUSIONS The stability of different types of osteosynthesis is almost equal - the most stable is the screw synthesis (24% probability of failure), the plates show 36% odds of failure, and the most unstable seem to be the C-nails with 42% probability of failure. However, these results do not correlate with the clinical conditions according to the A-H score. Key words: calcaneal fracture, locking plate, intramedullary nail.
