  • CONCLUSIONS: This case highlights the atypical presentation of NO, diagnostic workup, and surgical interventions. It emphasizes the need for a high index of suspicion in patients with unexplained, progressive mononeuropathies. When conventional imaging and electrodiagnostic studies are inconclusive, early referral for advanced imaging and specialist evaluation may be critical.   • CONCLUSIONS: In this qualitative study, surgical decisions among Black patients with CTS were associated with a complex interplay of physical, social, cultural, and financial factors. Culturally tailored shared decision-making that respects patient autonomy, acknowledges financial constraints, and integrates community perspectives may reduce disparities and improve surgical engagement.
