- CONCLUSIONS: In patients with pleural effusion and ADA levels ≥ 40 U/L, neither the cancer ratio nor the pleural LDH/ADA ratio alone provided sufficient discriminatory power between malignant pleural effusion and tuberculous pleurisy. The LAPAGE index may serve as a useful adjunctive tool in the differential diagnosis of tuberculosis and malignancy in pleural effusions with high ADA levels.

- CONCLUSIONS: FEF25-75% across the life course, particularly from early life, could be more relevant to COPD than previously recognised. Several childhood and novel parental preconception factors were associated with FEF25-75% trajectories, although causality cannot be inferred due to no clear temporality of these associations. Our findings on the relationship between FEF25-75% and COPD should be interpreted as life-course associations rather than as predictive. As COPD was rare in the average trajectory, the magnitude of effect estimates could be inflated.

- CONCLUSIONS: Closed pleural biopsy consistently has a low diagnostic yield and is not recommended as a first-line biopsy method for patients with undiagnosed exudative pleural effusions. Rigid medical thoracoscopy offers distinct clinical value for a complete visual inspection, large biopsies, and potential pleurodesis, particularly in suspected mesothelioma cases. The currently available evidence does not support the routine replacement of semirigid medical thoracoscopy with cryobiopsy. Computed tomography-guided biopsy may be preferred in cases involving thickening ≥10 mm or small target lesions. Conversely, ultrasound-guided biopsy may be appropriate for lesions measuring ≥3 mm with a clear acoustic window. UE-guided pleural biopsy shows promise as an emerging option, although the evidence is limited. In tuberculous pleurisy, medical thoracoscopy may offer higher diagnostic sensitivity in certain settings, particularly in high-prevalence areas. The current evidence is insufficient to establish a clear advantage for any biopsy method regarding the overall complication risk.

- CONCLUSIONS: GC and conventional IS currently remain the most evidence-based approaches for managing most cardiopulmonary manifestations in SLE. Despite aggressive immunosuppressive therapies, patients with some conditions, such as LM, ALP, and DAH, still experience high mortality. The quality of the existing evidence and the insights into novel therapies in these patients were limited, highlighting the urgent need for RCTs and optimized management in this area.

- CONCLUSIONS: The active ingredients of Shizao Decoction exert therapeutic effects on pleurisy by modulating T cell receptor and C-type lectin receptor signaling pathways through key targets (IL-6, TNF, IL-1β, STAT3, IL-2, NFKB1). These ingredients directly regulate pleurisy-specific pathophysiological processes, including pleural capillary permeability, inflammatory cell infiltration, and effusion formation, participate in immune responses, and exert anti-inflammatory and antiviral effects. The findings not only clarify the therapeutic mechanism of Shizao Decoction for pleurisy but also provide a theoretical basis for its clinical application and the development of new therapeutic strategies for pleurisy.
